肺结核伴糖尿病患者Th1/Th2细胞因子水平及意义

摘　　要：目的探讨肺结核伴糖尿病患者Th1/Th2细胞因子水平及意义。方法选取2014年4月至2016年3月在我院治疗肺结核合并糖尿病患者52例,单纯肺结核患者69例,同时选取60例健康志愿者作为对照组,检测各组外周血Th1、Th2细胞以及相关细胞因子水平。结果肺结核组和肺结核合并糖尿病组患者Th1均明显低于对照组(P<0.05),而Th2明显高于对照组(P<0.05);肺结核合并糖尿病组患者Th1为(6.21±1.95)%,明显低于肺结核组,而Th2为(5.81±1.01)%,明显高于肺结核组(P<0.05);肺结核组和肺结核合并糖尿病组患者Th1细胞因子均明显低于对照组(P<0.05),而Th2细胞因子均明显高于对照组(P<0.05);肺结核合并糖尿病组患者IFN-γ、IL-2和IL-12分别为(7.02±1.22)pg/mL、(11.21±2.15)pg/mL和(17.61±3.32)pg/mL,明显低于肺结核组(P<0.05);肺结核合并糖尿病组患者IL-4、IL-5和IL-6分别为(56.81±5.48)pg/mL、(47.87±3.39)pg/mL和(50.01±5.22)pg/mL,明显高于肺结核组(P<0.05)。结论相比较单纯肺结核,肺结核伴糖尿病患者Th1/Th2免疫失衡较重,免疫功能受损程度高。Objective To investigate the level of Th1/Th2 cytokines in patients with pulmonary tuberculosis complicated with diabetes mellitus and its significance.Methods From April 2014 to March 2016,52 cases of pulmonary tuberculosis complicated with diabetes mellitus(the group A),69 cases of simple pulmonary tuberculosis patients(the group B),and 60 healthy volunteers(the group C)were selected.Their peripheral blood Th1,Th2 cells and related cytokines levels were detected in each group.Results The level of Th1 was significantly lower in the group A and B than in the control group(P<0.05),while the level of Th2 was significantly higher in the group A and B than in the control group(P<0.05).The level of Th1 was(6.21±1.95)%in the group A,significantly lower than that in the group B,while Th2 was(5.81±1.01)%in the group A,significantly higher than that in the group B(P<0.05).The level of Th1 cytokines was significantly lower in the group A and B than in the control group(P<0.05),while the level of Th2 cytokines was significantly higher in the group A and B than in the control group(P<0.05).The levels of IFN-γ,IL-2 and IL-12 were(7.02±1.22),pg/mL(11.21±2.15)pg/mL and(17.61±3.32)pg/mL respectively in the group A,significantly lower than those in the group B(P<0.05).The levels of IL-4,IL-5 and IL-6 were(56.81±5.48)pg/mL,(47.87±3.39)pg/mL and(50.01±5.22)pg/mL in the group A,significantly higher than those in the group B(P<0.05).Conclusion Compare patients only with simple pulmonary tuberculosis,Th1/Th2 immune imbalance in pulmonary tuberculosis patients complicated with diabetes mellitus is more serious,and their immune function is severely damaged.
