摘 要:目的探讨支气管扩张合并感染患者血清和肽素表达水平及其临床意义。方法根据支气管扩张症的诊断标准,选取该类患者于感染期的100例为研究组,同时随机选取健康人50例作为对照组。采用ELISA双抗体夹心定量检测两组血清和肽素和降钙素原水平,并检测两组患者的炎症指标表达水平。结果研究组的血清和肽素表达水平、降钙素原表达水平、中性粒细胞比例、CRP水平均明显高于对照组,差异有统计学意义(P<0.05)。在研究组患者中,随着感染程度的加重,患者的血清和肽素水平、降钙素原水平、中性粒细胞比例、CRP水平逐渐升高,不同感染程度患者之间比较,差异有统计学意义(P<0.05)。研究组患者中,血清和肽素表达水平与降钙素原、中性粒细胞数及C-反应蛋白呈正相关(r值分别为0.73,0.64,0.59,均P<0.05)。结论血清和肽素水平检测可作为支气管扩张合并感染早期诊断的新型的生物学指标,联合检测降钙素原、中性粒细胞数及C-反应蛋白,有利于指导临床判断炎症反应以及病情发展状况。Objective To study the expression and its clinical significance of serum copeptin in patients with bronchiectasis and infection.Methods According to the diagnosis standard of bronchiectasis,100 patients with infection were selected for the study.50 cases of healthy people were randomly selected as the control group.The serum level of copeptin and procalcitonin(PCT)in the two groups were detected by ELISA,and the count of white blood cells and the levels of CRP were also detected.Results The serum levels of copeptin,CRP and PCT and the count of white blood cells were significantly higher in the study group than in the control group(P<0.05).In the study group,as the aggravation of infection,the serum levels of copeptin,PCT and CRP and the count of white blood cells increased with statistical significance(P<0.05).In the study group,the serum level of copeptin was positively correlated with the levels of PCT and CRP and the count of white blood cells(r=0.73,0.73,0.64,P<0.05).Conclusion The serum level of copeptin can be used as a new biological indicator in early diagnosis of bronchiectasis with infection,and the combined detection of PCT,CRP and white blood cells is helpful to guide clinical judgment on inflammation and disease development.
