膝三针、艾灸结合超激光照射治疗膝骨关节炎对下肢生物力学相关参数变化的影响

摘　　要：目的:观察膝三针、艾灸结合超激光照射治疗膝骨关节炎疗效及对下肢生物力学相关参数变化的影响。方法:将120例膝骨关节炎患者随机数字法分为对照组与观察组,每组60例。对照组单纯给予超激光照射治疗,观察组在对照组治疗基础上结合膝三针、艾灸治疗,记录治疗前后主要中医症状积分的变化情况和统计两组临床治疗总有效率,并测定膝关节支撑期内屈伸角度峰值和膝关节肌肉屈伸力矩峰值。结果:两组治疗后的关节疼痛、骨节肥大、胫膝酸软、屈伸不利症状积分均有显著降低(P<0.05),且观察组的各项症状积分显著低于对照组(P<0.05)。观察组治疗总有效率显著高于对照组(P<0.05)。两组治疗后触底瞬间最大屈角和支撑末期最大屈角显著小于治疗前(P<0.05),支撑中期最大屈角显著大于治疗前(P<0.05);且观察组的各项变化幅度比对照组显著(P<0.05)。两组治疗后的触底时间段最大伸膝肌肉力矩显著大于治疗前(P<0.05),且观察组变化幅度比对照组显著(P<0.05)。结论:膝三针、艾灸结合超激光治疗膝骨关节炎可有效改善膝痹的中医症状,疗效确切,并改善患者膝关节屈伸角度和肌肉屈伸力矩参数,有利于促进下肢生物力学平衡功能的恢复。Objective:To observe the effect of the combination therapy of knee tri-acupuncture,moxibustion,and super-laser irradiation on knee osteoarthritis,and to observe the changes of lower limb biomechanics by this therapy.Methods:120 cases of knee osteoarthritis were randomly divided into the control group and the observation group,with 60 cases in each group.The control group was treated with super-laser irradiation;on which basis,the observation group was also given knee tri-acupuncture and moxibustion.Scores of TCM symptoms before and after the treatment were recorded and the total effective rate was analyzed in the two groups.The peak of flexion and extension of the knee joint and the muscles were measured as well.Results:Scores of TCM symptoms,such as joint pain,joint hypertrophy,tibial debility,inhibited flexion and extension were reduced significantly in the two groups after the treatment(P<0.05),and the observation group was better than the control group(P<0.05).The total effective rate was significantly higher in the observation group than that in the control group(P<0.05).The maximum angle of flexion of the knee joint became smaller after the treatment in the two groups when feet touching ground and during late period of the knee joint support(P<0.05);it became larger during the middle period of knee joint support(P<0.05);the changes were more significant in the observation group than those in the control group(P<0.05).The torque of the stretching muscle was increased when feet touching ground after the treatment in the both groups,in which the observation group improved more(P<0.05).Conclusion:The combination therapy of knee tri-acupuncture,moxibustion,and super-laser irradiation can effectively improve the symptoms of knee osteoarthritis,it can improve the flexion and extension of the joint and the muscles,which is conducive to balance recovery of lower limbs from the aspect of biomechanics.
