二维及三维超声造影与常规超声诊断附件区囊性肿块的比较研究

摘　　要：目的探讨超声造影技术对附件区囊性灶的诊断优势及三维超声造影鉴别诊断炎性肿块和囊腺类肿瘤的价值。方法回顾性分析95个附件区囊性灶的常规超声、二维超声造影及三维超声造影影像,评估不同检查技术下不同囊肿的内部回声,囊壁显影及分隔、乳头等成像方面的差异。结果在判断病灶内部是否有活性组织成分及清晰显示囊壁方面超声造影优于常规的灰阶超声(P<0.05);与二维超声造影比较,三维超声造影在病灶内发现了更多的分隔和乳头(P<0.05);炎性肿块和囊腺类肿瘤的超声造影表现有差异(P<0.05);三维超声造影在显示肿块内部分隔的立体特征方面较二维超声造影更有优势(P<0.05)。结论超声造影技术较常规超声更能清晰地显示囊壁和囊内的真实回声,明确肿块的物理性质;在鉴别诊断炎性肿块和囊腺类肿瘤方面,三维超声造影提供了更多的诊断信息。ObjectiveTo investigate the enhancement features of adnexal cystic masses between three dimensional contrast enhanced ultrasonography(3D-CEUS)and two dimensional contrast enhanced ultrasonography(2D-CEUS).Methods95 lesions with ultrasound,2D-CEUS,3D-CEUS were retrospecively analyzed.Internal echo,cyctic wall,septa or the papillary parts of different masses on the three imaging modes were compared,the difference of inflammatory masses and cystadenoma or cystadenocarcinoma on 2D-CEUS and 3D-CEUS was compared.ResultsContrast enhanced ultrasound was superior to[JP2]general ultrasound in the display of intra echo,septa or papillary parts,cystic wall of cyst masses(P<0.05);The future of inflammatory masses and cystadenoma or cystadenocarcinoma on 2D-CEUS and 3D-CEUS was diffent(P<0.05);3D-CEUS was superior to 2D-CEUS in the display of intra septa or papillary parts(P<0.05).ConclusionContrast enhanced ultrasound is a useful technique in the diagnosis of physical property of lesions.3D-CEUS displays the overall feature of intra cyst,which provides additional information in the diagnosis.
