颅内压监护下脑室外引流联合腰穿治疗合并外伤性蛛网膜下腔出血的颅脑损伤效果观察

摘　　要：目的观察颅内压监护下脑室外引流联合腰穿治疗合并外伤性蛛网膜下腔出血的颅脑损伤临床效果。方法将170例合并外伤性蛛网膜下腔出血的颅脑损伤患者随机分为2组。治疗组91例在颅内压监护下行脑室外引流联合腰穿治疗,对照组79例行常规治疗。比较2组脑血管痉挛、脑梗死、脑疝、创伤性脑积水等并发症发生率和6个月格拉斯哥预后评分。结果治疗组脑血管痉挛、脑梗死、脑疝和创伤性脑积水发生率均低于对照组,差异有统计学意义(P<0.05),治疗组根据格拉斯哥预后评分恢复良好比率高于对照组,差异有统计学意义(P<0.05)。结论颅内压监护下行脑室外引流联合腰穿治疗能显著降低合并外伤性蛛网膜下腔出血的颅脑损伤患者并发症发生率,提高患者的生存质量。Objective To observe the trentment effect of intracranial pressure monitoring ventricle drainage combined with lumbar puncture in treatment of craniocerebral injury patients with traumatic subarachnoid hemorrhage patients.Methods One hundred and sevety patients with traumatic subarachnoid hemorrhage were randomly divided into two groups,91 patients with intracranial pressure monitoring ventricle drainage combined with lumbar puncture as the treatment group and 79 patients received conservative treatment as control group.To observe the incidence rate of two groups of patients with cerebral vasospasm,cerebral infarction,traumatic hydrocephalus,cerebral hernia and 6 months Glasgow Prognostic Scale were compared.Results The incidence of cerebral vasospasm,cerebral infarction,cerebral hernia and traumatic hydrocephalus in the treatment group was significantly lower than that in the control group(P<0.05).The treatment group Glasgow Prognosis Scale recovery rate was higher than those of control group,which had significant difference(P<0.05).Conclusion Intracranial pressure monitoring ventricle drainage combined with lumbar puncture can significantly reduce the incidence of complications and improve the quality of life of the craniocerebral injury patients with traumatic subarachnoid hemorrhage.
