寻常型银屑病合并代谢综合征与血清脂肪因子Chemerin的相关性研究

摘　　要：目的探讨寻常型银屑病合并代谢综合征(PSMS)患者血清脂肪因子Chemerin的水平变化及相关因素。方法选择2015年12月—2016年10月空军总医院收治的寻常型银屑病120例,将PSMS 60例作为观察组,寻常型银屑病60例作为对照组。记录两组一般资料,检测代谢指标、炎性因子及血清Chemerin水平,同时评估银屑病严重程度。结果观察组体重指数(BMI)、腰围、血压、空腹血糖、空腹胰岛素、胰岛素抵抗指数、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)水平均高于对照组,高密度脂蛋白胆固醇(HDL-C)水平低于对照组(P<0.05)。观察组超敏C反应蛋白(hs-CRP)、白介素-6(IL-6)、银屑病皮损面积和严重程度指数(PASI)评分、血清Chemerin水平高于对照组(P<0.05)。观察组血清Chemerin水平与BMI、腰围、TG、LDL-C、hs-CRP、IL-6及PASI呈正相关(P<0.05)。TG、腰围、hs-CRP、IL-6是血清Chemerin水平的影响因素(P<0.05)。结论血清Chemerin通过参与炎症过程并影响PSMS的发病及进展,且其血清水平与患者肥胖、脂代谢紊乱、炎症及银屑病严重程度密切相关。Objective To investigate change of serum adipokines Chemerin level and its related factors in patients with psoriasis vulgaris combined with metabolic syndrome(PSMS).Methods A total of 120 patients with psoriasis vulgaris admitted during December 2015 and October 2016 were divided into observation group(n=60,PSMS patients)and control group(n=60,psoriasis vulgaris patients).In two groups,general informations were recorded,and metabolic indexes,levels of inflammatory factors and Chemerin levels were detected,and severe degrees of psoriasis were evaluated simultaneously.Results In observation group,levels of body mass index(BMI),waist circumference(WC),blood pressure(BP),fasting plasma glucose(FPG),fasting insulin(Fins),insulin resistance index,triglyceride(TG),total cholesterol(TC)and low density lipoprotein cholesterol(LDL-C)were significantly higher,while high density lipoprotein cholesterol(HDL-C)level was significantly lower than those in control group(P<0.05);levels of high sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),damaged skin area of psoriasis,psoriasis area and severity index(PASI)score and serum Chemerin level were significantly higher than those in control group(P<0.05).Serum Chemerin level was positively correlated with BMI,WC,TG,LDL-C,hs-CRP,IL-6 and PASI in observation group(P<0.05).TG,WC,hs-CRP and IL-6 were independent factors of serum Chemerin level(P<0.05).Conclusion Serum Chemerin may affect PSMS pathogenesy and development by involving inflammation process,and its level is closely correlated with obesity,lipid disorders,inflammation and severe degree of psoriasis.
