自拟化瘀定痛方联合依托考昔对急性痛风性关节炎患者相关炎性因子的影响

摘　　要：目的观察自拟化瘀定痛方联合依托考昔对急性痛风性关节炎患者相关炎性因子的影响。方法将78例急性痛风性关节炎患者随机分为2组,对照组39例给予依托考昔片120 mg/次口服,1次/d;观察组在对照组治疗基础上给予自拟化瘀定痛方口服,1剂/d,水煎分早晚服。分别于治疗前及治疗2周后检测2组血尿酸(UA)、红细胞沉降率(ESR)、肌酐(Cr)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、环氧化酶-2(COX-2)水平,并采用视觉模拟疼痛评分法(VAS)和肿胀评分评定患者疼痛和关节肿胀情况。结果 2组治疗后血UA、Cr、ESR、IL-1β、IL-6、TNF-α、COX-2水平均明显低于治疗前(P均<0.05),且观察组治疗后各指标水平均明显低于对照组(P均<0.05);2组治疗后VAS评分、关节肿胀评分均明显低于治疗前(P均<0.05),且观察组治疗后VAS评分、关节肿胀评分均明显低于对照组(P均<0.05)。结论自拟化瘀定痛方联合依托考昔治疗急性痛风性关节炎更有利于促进关节疼痛缓解及功能恢复,可能与调控血UA、IL-1β、IL-6、TNF-α、COX-2水平来抑制炎症反应有关。Objective It is to explore the effect of self-made Huayu Dingtong recipe combined with etoricoxib on the related inflammatory cytokines in patients with acute gouty arthritis(GA).Methods A total of 78 patients with acute GA were randomly divided into the observation group(n=39)and the control group(n=39).The patients in the control group were given etoricoxib by oral administration,120 mg/time,1 time/d.Additionally,the patients in the observation group were given self-made Huayu Dingtong recipe based on the treatment in the control group,1 dose/d,decoction with water,taking in the morning and evening with warm water.The efficacy was evaluated after 2-week treatment.The morning fasting peripheral venous blood before and after treatment in the two groups was collected.HPLC was used to detect UA.Westergren was used to detect ESR.The full automatic biochemical analyzer was used to detect Cr.ELISA was used to detect IL-1β,IL-6,TNF-α,and COX-2.VAS and swelling grading were used to evaluate the improvement of symptoms.Results The levels of UA,Cr,ESR,IL-1β,IL-6,TNF-αand COX-2 after treatment in the two groups were significantly lower than those before treatment,and the levels of these indexes in the observation group after treatment were lower than those in the control group(P<0.05).The VAS scores and joint swelling scores after treatment in the two groups were significantly lower than those before treatment,and the scores in the observation group after treatment were significantly lower than those in the control group(P<0.05).Conclusion Self-made Huayu Dingtong recipe combined with etoricoxib can contribute to alleviate the joint pain and promote the recovery of joint function in the treatment of acute GA,whose mechanism is associated with inhibition of inflammatory reaction through regulating the levels of UA,IL-1β,IL-6,TNF-αand COX-2.
