通痹止痛方结合针刺治疗根性坐骨神经痛疗效及对β-内啡肽、炎性细胞因子水平的影响

摘　　要：目的观察通痹止痛方结合针刺治疗根性坐骨神经痛疗效及对β-内啡肽、炎性细胞因子水平的影响。方法将60例根性坐骨神经痛患者随机分为2组,对照组30例给予双氯芬酸二乙胺乳胶治疗,观察组30例在此基础上加用通痹止痛方结合针刺治疗,观察2组治疗前后主要症状评分、疼痛视觉模拟评分(VAS)、日本骨科协会下腰痛评价量表评分(JOA)、Oswestry功能障碍指数评分(ODI)及血清β-内啡肽(β-EP)、白细胞介素-1(IL-1)、IL-6及肿瘤坏死因子-α(TNF-α)水平变化情况,统计2组近期疗效和不良反应发生情况。结果 2组治疗后主要症状评分、VAS评分、ODI评分及IL-1、IL-6、TNF-α水平均显著降低(P均<0.05),JOA评分和β-EP水平均显著提高(P均<0.05),且观察组治疗后以上指标改善情况均显著优于对照组(P均<0.05);观察组治疗总有效率显著高于对照组(P<0.05);2组不良反应发生率比较差异无统计学意义(P>0.05)。结论通痹止痛方结合针刺治疗根性坐骨神经痛可有效改善相关症状体征,降低疼痛程度,提高肢体活动功能,且安全。Objective It is to observe the therapeutic effect of Tongbi Zhitong Recipe combined with acupuncture on radical sciatica and its effect on the level ofβ-endorphin and inflammatory cytokines.Methods Sixty patients with radical sciatica were randomly divided into two groups.The control group(n=30)was treated with diclofenac diethylamine emulsion.The observation group(n=30)was treated with Tongbian Zhitong Recipe combined with acupuncture based on the treatment in the control group.The changes of main symptom scores,VAS score,JOA score,Oswestry Disability Index score(ODI)and serumβ-endorphin(β-EP),interleukin(IL-1),IL-6 and tumor necrosis factor-α(TNF-α)were observed in both groups.The curative effect and adverse reactions of two groups were calculated.Results The main symptom scores,VAS scores,ODI scores and the levels of IL-1,IL-6 and TNF-αwere significantly decreased in both groups(all P<0.05),JOA scores andβ-EP level were significantly increased after treatment(P<0.05),and the improvements of the above indexes in the observation group after treatment were significantly better than those in the control group(all P<0.05);the total effective rate in the observation group was significantly higher than that in the control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Tongbi Zhitong Recipe combined with acupuncture can effectively improve the symptoms and signs,reduce the degree of pain,improve limb mobility with good safety in the treatment of radical sciatica.
