CYP2C19基因多态性与浙江汉族老年冠心病的相关性研究

摘　　要：目的探讨细胞色素P450 2C19(CYP2C19)基因多态性与浙江汉族老年人群冠心病的相关性。方法采用基因芯片技术检测210例冠心病患者和206例对照组的基因多态性,对两组患者基本临床资料、实验室指标、CYP2C19基因分布进行分析。结果冠心病组及对照组不同基因型和等位基因分布差异有统计学意义(χ2分别=10.53、6.48,P均<0.05),且冠心病组CYP2C19*2、CYP2C19*3等位基因频率明显高于对照组,差异有统计学意义(χ2分别=15.36、6.00,P均<0.05)。logistic回归分析校正性别、年龄、体重指数、血压、总胆固醇、低密度脂蛋白胆固醇等传统危险因素之后,血糖高、有吸烟史、携带CYP2C19*2、CYP2C19*3等位基因患冠心病风险明显增加(OR分别=1.90、1.28、2.80、3.57,P均<0.05)。结论携带CYP2C19*2或CYP2C19*3基因是浙江汉族老年人群冠心病发生的重要危险因素。CYP2C19基因多态性会增加浙江汉族老年人群患冠心病的发生风险。Objective To investigate the association between CYP2C19 gene polymorphism and coronary heart disease(CHD)in elderly of Han nationality in Zhejiang.Methods The CYP2C19 genotypes of 210 patients with CHD and 206 cases of control were detected by using gene chip technology.The basic clinical data,laboratory results,polymorphisms distribution of CYP2C19 were retrospectively analyzed.Results There were statistical differences in the genotypes and frequencies of alleles of CYP2C19 between the two group(χ2=10.53,6.48,P<0.05),and the frequencies of CYP2C19*2,CYP2C19*3 in CHD group were higher than those in the controls(χ2=15.36,6.00,P<0.05).Adjusting for traditional cardiovascular factors including gender,age,BMI,hypertension,TC and LDL-C,the logistic regression analysis showed that the history of smoking,higher blood sugar,CYP2C19*2,CYP2C19*3 were significant risk factors of CHD(OR=1.90,1.28,2.80,3.57,P<0.05).Conclusion CYP2C19*2 or CYP2C19*3 may be the independent risk factors of CHD.CYP2C19 gene polymorphism can increase the risk of coronary heart disease in the elderly of Han nationality in Zhejiang.
