血小板和淋巴细胞比率与高血压脑出血的关系

摘　　要：目的预测血小板/淋巴细胞比率(PLR)对高血压脑出血的诊断价值,探讨PLR水平与高血压脑出血的关系。方法选取高血压脑出血患者200例(研究组),健康正常者200例(健康组),计算并比较2组PLR的水平。用ROC曲线预测PLR对高血压脑出血的诊断价值,将研究组中PLR<cut off值的患者归为PLR低值组(低值组)、PLR≥cut off值的患者归为PLR高值组(高值组),并比较它们的相关指标。结果研究组PLR高于健康组[(171.0±72.5)比(112.3±53.5),P<0.05]。用ROC曲线结果显示曲线下面积(AUC)为0.897(95%CI0.852~0.942,P<0.05),cut off值为154.642,敏感性84%,特异性84%。高值组(n=89)较低值组(n=114)年龄更大,贫血发生率、心梗发生率、吸烟率、血小板和中性粒细胞的计数、住院及随访1年病死率也更高,淋巴细胞计数更低(均P<0.05)。结论 PLR对高血压脑出血患者有较高的诊断价值,且是预后不良的独立危险因素。Objective To predict the diagnostic value of platelet/lymphocyte ratio(PLR)in hypertensive cerebral hemorrhage,and to investigate the relationship between them.Methods The levels of PLR were measured in 200 patients with hypertensive cerebral hemorrhage(study group)and 200 healthy subjects(healthy group).ROC curve was used to predict the diagnostic value of PLR in hypertensive cerebral hemorrhage.Patients in study group were further divided into low-value group(PLR<cut off value)and high-value group(PLR≥cut off value).Clinical pathological characteristics were compared between the two groups.Results The PLR in study group(171.0±72.5)was higher than that in healthy group(112.3±53.5)(P<0.05).The area under the curve(AUC)value was 0.897(95%CI,0.852-0.942)(P<0.05).The cut off value,sensitivity and specificity were 154.642,84%and 84%,respectively.Compared with low-value group(n=114),the age,anemia incidence,myocardial infarction incidence,smoking rate,platelet count,neutrophils count and in-hospital and 1-year mortality rates increased but lymphocyte count decreased in high-value group(n=89)(P<0.05).Conclusion PLR has a high diagnostic value and is an independent risk factor for poor prognosis in hypertensive cerebral hemorrhage.
