门诊2型糖尿病合并高血压患者生存质量及其影响因素分析

摘　　要：目的调查门诊2型糖尿病合并高血压患者的生存质量,分析影响因素。方法采用一般资料调查表、世界卫生组织生存质量测定量表简表(WHOQOL-BREF)对在江苏省南京市某三甲医院门诊就诊的158例2型糖尿病合并高血压患者进行调查。结果 158例患者生存质量测定在生理、心理、社会关系及环境四个领域得分分别为(14.87±2.50)分、(14.28±2.50)分、(14.69±5.19)分和(14.27±2.47)分,总的生存质量和健康状况得分分别为(3.42±0.87)分和(2.83±0.88)分。影响生理领域得分的因素有是否定期做检查;影响心理领域得分的因素有体质指数(BMI)、低血糖发生及合并其他疾病情况;影响社会关系得分的因素有婚姻状况、糖尿病治疗方式、是否定期做检查及失眠情况;影响环境领域得分的因素有婚姻状况、低血糖发生及失眠情况;是否合并其他疾病影响患者总的生存质量;失眠情况影响总的健康状况。结论门诊2型糖尿病合并高血压患者的生存质量较好。今后需对合并多种疾病、定期检查、肥胖患者加强心理干预;对失眠患者进行干预,提高睡眠质量;加强低血糖防治方面的糖尿病教育,防止低血糖发生。从而促进患者生理、心理健康,社会关系及环境改善,全面提高生存质量。Objective To investigate and analyze influencing factors of the life quality among the outpatients with type 2 diabetes and hypertension.Methods A total of 158 outpatients from a tertiary hospital in Nanjing were investigated using by a self-designed questionnaire and World Health Organization Quality of Life-BREF(WHOQOL-BREF).Results According to the scale of WHOQOL-BREF,scores of physiological,psychological,social,and environmental dimension were(14.87±2.50),(14.28±2.50),(14.69±5.19),and(14.27±2.47),respectively.The scores of general life quality and health condition were(3.50±0.86)and(2.90±0.89),respectively.The influencing factor of physiological dimension was whether checking-up regularly.The influencing factors of psychological dimension were BMI,hypoglycemia,combined diseases.The influencing factors of social dimension were marital status,diabetes treatment,whether checking up regularly and the insomnia.The influencing factors of environmental dimension were marital status,hypoglycemia and insomnia.General quality of life was affected by complicating diseases,and the general health was affected by insomnia.Conclusion The overall quality of life of outpatients with type 2 diabetes and hypertension is relatively sound.It is necessary for the doctors and nurses to pay more attention on the patients combined with other diseases,hypoglycemia,and insomnia to improve their QOL comprehensively.
