牵引锻炼颈托在颈型颈椎病治疗中的应用效果

摘　　要：目的探讨静力牵引下使用牵引锻炼颈托行等长收缩锻炼治疗颈型颈椎病的临床效果。方法将2016年7月至2017年1月在佛山市中医院门诊就诊的56例颈型颈椎病患者随机分为治疗组(29例)和对照组(27例),对照组采用常规颈后肌群等长收缩锻炼,治疗组在牵引状态下使用牵引锻炼颈托进行颈部肌群等长收缩锻炼,疗程均为2周。采用视觉模拟量表(VAS)评分和Vernon颈椎废用指数(VCDI)评价两组患者的临床疗效。结果经随访3~6个月,治疗组患者病情均无进展,对照组进展为神经根型颈椎病和交感型颈椎病各1例。与治疗前相比,治疗后两组VAS评分、VCDI均显著降低,治疗组降幅较对照组更为显著(P<0.05);治疗组3个月内颈椎病发作次数明显低于对照组(P<0.05);安全性评价方面,治疗组6例患者首次治疗结束后下颌部出现紫红色痕迹,次日逐渐消退,两组患者均未出现骨折、昏厥等不良反应。结论借助牵引锻炼颈托在牵引状态下行等长收缩锻炼治疗颈型颈椎病安全有效,操作简便,是颈型颈椎病保守治疗的新选择。Objective To evaluate the clinical application of isometric contraction training(ICT)under static traction using traction exercise cervical gear(TECG)for cervical spondylopathy.Methods A total of 56 outpatients with cervical spondylopathy treated from July 2016 to January 2017 in Foshan Hospital of TCM,were recruited and divided into treatment group(n=29)and control group(n=27)randomly.Patients in treatment group were treated by ICT with TECG under static traction,and patients in control group were only treated via routine ICT on the posterior cervical muscles.All patients were treated for two weeks.After the treatment,curative effects were evaluated based on visual analogue scale(VAS)score and Vernon cervical disability index(VCDI).Results After 3 to 6 months'follow-up,there was no progress of disease in treatment group,while in control group,one case developed to cervical spondylotic radiculopathy,and one sympathetic cervical spondylosis respectively.After treatment,VAS score and VCDI in two groups all decreased significantly(P<0.05),and the decrease in treatment group was more remarkable(P<0.05).During 3 months'follow-up,the episodes of attack in treatment group were lower than those in control group(P<0.05).As for safety evaluation,6 patients in treatment group were observed purple skin trace at lower jaw after the first treatment,which gradually resolved on the following days;No patients were found fracture,syncope or other adverse effects.Conclusion ICT with TECG under traction is a new choice of conservative treatment for cervical spondylopathy,which has the advantages of safety,effectiveness and easy operation.
