抗旋型股骨近端髓内钉和动力髋螺钉治疗老年人股骨转子间骨折的比较研究

摘　　要：目的 :对采用抗旋型股骨近端髓内钉(resistance to rotation of proximal femoral nail,PFN-A)及动力髋螺钉(dynamic hip screw,DHS)治疗老年股骨转子间骨折的手术资料和术后资料进行比较。方法 :选取144例股骨转子间骨折高龄患者,给予DHS内固定术75例和PFN-A内固定术69例,记录两组术中、术后的多项相关指标并予以统计学分析。结果:患者经过1~2年随访达到骨折临床愈合,平均16个月,因内科疾病死亡5例;PFN-A组与DHS组切口长度、手术时间、术中失血量、术中X线照射时间等术中一般情况,术后并发症比较差异均具有统计学意义。结论:PFN-A治疗老年股骨转子间骨折,手术创伤小,固定可靠,术后并发症少。Objective:To compare the data of operation during and after operation,using of resistance to rotation of proximal femoral nail(PFN-A)and dynamic hip screw(DHS)in treatment of intertrochanteric fractures in the elderly.Methods:144 cases of elderly patients with intertrochanteric fracture were enrolled.75 cases were fixed by using DHS;69 cases with PFN-A internal fixation.Record two groups of intraoperative and postoperative related indicators and to be statistically analyzed.Results:All patients were followed up for 1-2 years to fracture healing,an average of 16 months,5 cases died of internal medicine diseases.PFN-A group and DHS group were analyzed statistically.The length of incision,operation time,intraoperative blood loss,intraoperative X line irradiation time,postoperative complications showed statistically significant differences.Conclusion:PFN-A internal fixation has small operation wound,the characteristics of strong stability,less postoperative complications,suitable for elderly patients with femoral intertrochanteric fracture.
