极低出生体重儿早期静脉营养的应用与价值探析

摘　　要：目的探讨极低出生体重儿早期静脉营养的应用与价值。方法方便选取2016年2月—2017年1月该院收治的90例极低出生体重儿作为对象,根据随机信封法分组,各有45例。晚期静脉营养组采用晚期静脉营养支持,时间为出生后72 h后,早期静脉营养组采用早期静脉营养支持,时间为出生后24 h内。比较2组干预结果。结果早期静脉营养组并发症发生率4.44%低于晚期静脉营养组22.22%(χ2=5.244,P<0.05)。早期静脉营养组身长每周增长情况、头围每周增长情况分别(0.95±0.07)cm/周,(0.69±0.21)cm/周,高于晚期静脉营养组(0.51±0.03)cm/周、(0.51±0.11)cm/周。早期静脉营养组体重达到2 kg的时间、静脉营养支持天数(29.62±1.21)d、(24.51±1.61)d,短于晚期静脉营养组(34.45±4.57)d和(30.61±2.59)d,羌率有统计学意义(P<0.05)。结论极低出生体重儿早期静脉营养的应用与价值高,可有效改善血红蛋白、白蛋白、血钙水平、热量摄入情况,提高身长、头围每周增长幅度,增加体重,缩短静脉营养支持天数,减少并发症的发生,对极低出生体重儿预后有益,值得推广。Objective This paper tries to explore the application and value of early intravenous nutrition in very low birth weight infants.Methods 90 cases of very low birth weight infants from February 2016 to January 2017 were convenient selected and randomly divided into two groups according to the random envelope m ethod,with 45 cases in each group.Late venous nutrition group with advanced intravenous nutrition support,for 72 h after birth,and the early intravenous nutrition group used early intravenous nutritional support,for 24 h after birth.The results of the two groups were com pared.Results The incidence of com plications in early intravenous nutrition group was 4.44%,lower than that in advanced intravenous nutrition group of 22.22%,(字2=5.244,P<0.05).The growth rate of body height and head size in the early venous nutrition group were(0.95±0.07)cm/weeks,(0.69±0.21)cm/weeks,higher than that of advanced intravenous nutrition group of(0.51±0.03)cm/weeks,(0.51±0.11)cm/weeks.W hen the duration of intravenous nutrition reached to 2 kg,the intravenous supporting days were(29.62±1.21)d and(24.51±1.61)d,shorter than that of the late intravenous nutrition group of(34.45±4.57)d,(30.61±2.59)d,the differen was satisticaly significant(P<0.05).Conclusion The application and value of early intravenous nutrition in very low birth weight infants can effectively improve hemoglobin,album in,serum calcium level,calorie intake,increase the length,head circum ference growth rate,increase body weight,and shorten the nutritional support,reduce the incidence of com plications,which is beneficial for the very low birth weight children,therefore it is worth promotion.
