妊娠期肝内胆汁淤积患者总胆汁酸水平与胎儿宫内窘迫的相关性分析

摘　　要：目的探讨妊娠期肝内胆汁淤积患者总胆汁酸水平与胎儿宫内窘迫的相关性分析。方法选取2015年1月~2016年1月在我院接受诊断及治疗的妊娠期肝内胆汁淤积患者160例为观察组研究对象,结合患者总胆汁酸水平将其分为观察1组(102例)及观察2组(58例),以同期在我院实施体检的健康产妇160例为对照组,对比三组患者胎儿宫内窘迫的发生率,分析其生化指标检测结果。结果观察组患者在胎儿宫内窘迫发生率(28.1%)上与对照组患者(0.05%)相比存在显著性差异且高于对照组(P<0.05),同时观察组患者总胆汁酸水平与对照组相比存在显著性差异且远高于对照组(P<0.05)。此外,观察1组、观察2组患者总胆汁酸水平相比存在显著性差异且观察1组高于观察2组(P<0.05),观察1组(34.3%)、观察2组患者(17.2%)在胎儿宫内窘迫发生率上也存在显著性差异,且观察1组高于观察2组(P<0.05)。结论妊娠期肝内胆汁淤积患者胎儿宫内窘迫发生率显著高于健康产妇,同时患者总胆汁酸水平越高则胎儿宫内窘迫发生率越高。Objective To investigate the correlation between total bile acid level and fetal distress in patients with intrahepatic cholestasis during pregnancy.Methods A total of 160 pregnancy patients with intrahepatic cholestasis were enrolled in this study from January 2015 to January 2016.All subjects were divided into the observation group A(n=102)and B(n=58)according with the total bile acid levels.160 cases of healthy women were selected as the control group.The incidence of fetal distress was compared between the three groups,and the biochemical indexes were analyzed.Results There was statistical difference in the incidence of fetal distress between the observation group and the control group(28.1%vs 0.05%,P<0.05).The levels of total bile acid in the observation group were significantly higher than those in the control group(P<0.05).The levels of total bile acid in the observation group A were higher than the observation group B(P<0.05).The incidence of fetal distress in the observation group A was higher than the observation group B(34.3%vs 17.2%,P<0.05).Conclusion Compared with healthy women,pregnant patients with intrahepatic cholestasis has a high incidence of intrauterine distress.The higher the total bile acid levels,the higher the incidence of fetal distress.
