闭合复位经皮螺纹骨圆针内固定治疗儿童肱骨近端骨折的临床观察

摘　　要：目的观察闭合复位经皮穿螺纹骨圆针治疗儿童肱骨近端骨折的临床疗效。方法本组44例,采用手法闭合复位,经皮螺纹骨圆针穿针内固定治疗,采用Constant-Murley评分方法评定疗效。结果本组手术时间30-75min,平均40min。44例均进行随访6~12周,未发现骨折延迟愈合或不愈合。有两例出现进针处感染,骨折愈合后拔出骨圆针,经换药治疗,1周后愈合,无退针现象。按照Constant-Murley评分评价:优37例,良5例,可2例,优良率为95.45%。结论闭合复位经皮穿螺纹骨圆针内固定治疗儿童肱骨近端骨折具有手术创伤小,骨折愈合快,固定可靠,并发症少,费用低等优点,值得借鉴。Objective To observe the clinical effect of closed reduction and percutaneous threaded bone ring needle in the treatment of proximal humerus fracture in children.Methods Forty-four cases with closed reduction and percutaneous screw fixation for the treatment using bone round needle.Curative effect was evaluated by Constant-Murley scoring method.Results The operation time was 30-75min,with an average 40min.All 44 cases were followed up for 6~12 weeks without any delayed union or nonunion.A needle at the infection occurred in two cases,bone round needle after fracture healing was taken out.After dressing change treatment,fracture was healed after 1 week,with no withdrawal phenomenon.According to the Constant-Murley score,37 cases were excellent;5 cases were good;2 cases were acceptable.The excellent and good rate was 95.45%.Conclusion Closed reduction and percutaneous thread fixation with bone round needle in the treatment of proximal humerus fractures in children had the advantages of less surgical trauma,fast fracture healing,reliable fixation,less complications and lower cost,which was worthy of reference.
