超声评分量表在分娩前胎盘植入类型预测诊断中的应用

摘　　要：目的:探讨超声评分量表在分娩前胎盘植入类型预测诊断中的应用价值。方法:选取经临床证实为胎盘植入患者26例作为研究对象,回顾性分析临床资料。结果:26例患者均经术后病理证实为子宫瘢痕部位胎盘植入,分娩前经腹超声检查中应用超声评分量表评估,结果提示胎盘植入26例,其中粘连型20例,植入型4例,穿透型2例;根据患者术后临床及病理结果提示胎盘植入26例,其中粘连型23例,植入型2例,穿透型1例;提示胎盘植入的超声诊断符合率为100%,各类型预测符合率为粘连型86.9%、植入型50%、穿透型50%。结论:超声评分量表在分娩前超声检查中的应用,有助于明确胎盘植入的诊断,并且能够较好的预测胎盘植入不同类型,以指导后续临床治疗。Objective:To investigate the value of ultrasonic score system for predicting the type of placenta accreta before delivery.Method:Selected 26 patients as the research object who clinically confirmed for the placenta accreta,and made a retrospective analysis of the clinical data.Results:26 patients in this group were histologically confirmed as placenta accreta in the scar of uterus.Ultrasound score before transabdominal sonography showed that 26 cases were placenta accreta,including 20 cases of adhesive type,4 cases of implanted type and 2 cases of penetrating type;According to the patient's clinical and pathological findings suggest that there were 26 cases of placenta accreta,including 23 cases of adhesive type,2 cases of implanted type,1 case of penetrating type;The coincidence rate was 100%.The coincidence rate of various types of prediction was 86.9%for adhesive type,50%for implanted type and 50%for penetrating type.Conclusion:The application of ultrasonic score system in prenatal echocardiography can help to make a clear diagnosis of placenta accreta as well as predict different types of placenta accretion to guide the follow-up clinical treatment.Therefore,the ultrasonic score system worthy of promotion and application.
