免疫三氧自体血回输联合超声引导三氧椎旁注射治疗带状疱疹后神经痛疗效观察

摘　　要：目的观察免疫三氧自体血回输联合超声引导三氧椎旁注射治疗带状疱疹后神经痛(postherpetic neuralgia,PHN)的治疗效果。方法 84例年龄>60岁的胸腰段PHN患者,随机分为试验组及对照组,每组42例。对照组采用口服普瑞巴林和阿米替林,试验组口服对照组药物的基础上,给予免疫三氧自体血回输联合超声引导三氧椎旁注射治疗,并对2组患者分别于治疗前、治疗1个、2个疗程后进行视觉模拟评分(VAS评分)、神经损伤修复程度、生活质量改善情况进行观察。结果 2组患者在治疗后VAS评分均小于治疗前(P<0.05);经1个疗程治疗,试验组患者的VAS评分低于对照组(P<0.05)、经2个疗程治疗后患者的VAS评分显著低于对照组(P<0.01);试验组神经损伤修复程度总有效率明显高于对照组(P<0.05);2组患者的生活质量治疗后均优于治疗前(P<0.05),试验组与对照组相比更能提高患者的生活质量(P<0.05)。结论治疗老年慢性带状疱疹后神经痛,免疫三氧自体血回输联合超声引导三氧椎旁注射是一种安全有效的治疗方法,可明显减轻患者的疼痛,有助于神经损伤修复,提高患者的生活质量。Objective To observe the therapeutic effects of immune three oxygen autologous blood transfusion combined with ultrasound guided three oxygen paravertebral injection on postherpetic neuralgia(PHN).Methods Eighty-four patients with thoracolumbar PHN who were older than 60 years were randomly divided into experimental group(n=42)and control group(n=42).The patients in control group were treated by oral pregabalin and amitriptyline,however,the patients in experimental group were treated,on the basis of control group,were treated by immune three oxygen autologous blood transfusion combined with ultrasound guided paravertebral three oxygen paravertebral injection.The visual analogue scale(VAS)scores,recovery degree of nerve injury,improvement situation of life quality before treatment and after one treatment course and two treatment courses were observed and compared between two groups.Results After treatment the scores of VAS were significantly decreased in both groups,as compared with those before treatment(P<0.05).After one treatment course and after two treatment courses the VAS scores in experimental group were signifiacntly lower than those in control group(P<0.05 or P<0.01).The total effective rate in experimental group was 96.33%,which was significantly higher than that(74.59%)in control group(P<0.05).In addition the life quality of patients in both groups was superior to that before treatment(P<0.05),moreover,which in experimental group was superior to that in control group(P<0.05).Conclusion The immune three oxygen autologous blood transfusion combined with ultrasound guided three oxygen paravertebral injection is a safe and effective method in treatment of senile postherpetic neuralgia,which can obviously alleviate the pain of the patients,and is helpful to repair the nerve injury and improve the life quality of patients.
