舒适型护理在肝癌介入患者中的应用

摘　　要：目的探讨舒适护理在肝癌介入护理中的应用。方法选择100例符合纳入与排除标准的肝癌介入治疗患者随机分为观察组和对照组,每组50例。观察组以舒适护理理念为指导,采用使患者在一系列舒适的优化护理措施中接受治疗,包括缩短术前准备时间,规范接诊流程,预防性给予止吐药物,缩短穿刺部位压迫时间,缩短绝对卧床及肢体制动时间,缩短术后禁食水时间。对照组采用常规方法进行护理。观察患者介入后6、12、24 h穿刺部位出血情况以及患者对治疗的依从性情况,观察患者腰背部酸痛发生情况及缓解程度,尿潴留发生情况,消化道症状的发生情况。结果观察组能有效改善患者腰背部酸痛,降低尿潴留的发生,有效缓解消化道症状与对照组比较差异有统计学意义(P<0.05);观察组在介入治疗过程中治疗依从性与对照组比较差异有统计学意义(P<0.05),2组穿刺部位出血的发生情况比较差异无统计学意义(P>0.05)。结论以舒适护理理念为指导,运用循证医学的科学方法为肝癌介入治疗患者提供以患者舒适为目的的护理措施,有效的降低了患者因治疗造成的不舒适感,提高了患者的生活质量,促进身心健康,提高患者对介入治疗的依从性,同时未增加介入治疗并发症的发生。Objective To investigate the application of comfortable nursing in interventional nursing care of liver cancer.Methods One hundred patients with liver cancer who were accordant with interventional therapy criteria were randomly divided into observation group and control group,with 50 patients in each group.The patients in observation group were treated by comfortable nursing measures including shortening preoperative preparation time,standard process,prophylactic antiemetic drugs,shortening puncture compression time,shortening the absolute bed and limb braking time,shortening the postoperative fasting time.However the patients in control were treated by routine nursing measures.The puncture site bleeding status at 6h,12h 24h after intervention,the compliance of patients,incidence of lower back pain and remission degree,incidence rates of urinary retention and gastrointestinal symptoms were observed and compared between two groups.Results The incidence rates of lower back pain and urinary retention were obviously decreased in observation group,and gastrointestinal symptoms were significantly improved,as compared with those in control group(P<0.05).Moreover there was significant difference in treatment compliance during interventional therapy between the two groups(P<0.05).However there was no significant difference in puncture site bleeding status between two groups(P>0.05).Conclusion The application of comfortable nursing in interventional nursing care of liver cancer can effectively relieve uncomfortable feelings caused by interventional therapy,which can improve life quality of patients,promote physical and mental health of patients,enhance the compliance of patients to interventional therapy,without increasing the incidence of complications.
