脐针联合远红外理疗治疗膝骨关节炎的临床疗效及对血清肿瘤坏死因子α、基质金属蛋白酶3的影响

摘　　要：目的观察脐针联合远红外理疗对膝骨关节炎(KOA)患者血清肿瘤坏死因子(TNF-α)、基质金属蛋白酶3(MMP-3)的影响及临床疗效。方法将90例KOA患者随机分为2组。对照组45例予硫酸氨基葡萄糖钾胶囊口服治疗;治疗组45例在对照组治疗基础上加脐针及远红外理疗治疗。2组均30 d为1个疗程,治疗3个疗程后统计临床疗效。比较2组治疗前后血清TNF-α、MMP-3水平及关节症状评分、20 m行走时间变化。结果治疗组临床疗效优于对照组(P<0.05)。2组治疗后血清TNF-α及MMP-3水平均较本组治疗前明显降低(P<0.05),但2组组间比较差异无统计学意义(P>0.05)。2组治疗后关节疼痛、关节肿胀、关节功能障碍及关节压痛评分均较本组治疗前降低(P<0.05),20 m行走时间减少(P<0.05),且2组治疗前后差值比较差异亦均有统计学意义(P<0.05),治疗组大于对照组。结论脐针联合远红外理疗治疗KOA能有效抑制炎症细胞因子的渗出,促进软骨合成修复,有效缓解关节疼痛,改善关节活动功能,延缓病情进展。Objective To observe the effects of umbilical needle combined with far infrared physiotherapy on serum tumor necrosis factor-α(TNF-α)and matrix metalloproteinase-3(MMP-3)in patients with knee osteoarthritis(KOA).Methods 90 patients with KOA were randomly divided into two groups,45 cases in control group were treated by glucosamine potassium sulfate capsules,45 cases in treatment group were treated by umbilical needle combined with far infrared physiotherapy on the basis of control group treatment.Both groups were treated 30 days for a course,treated for 3 courses,the clinical efficacy were observed after treatment.The serum TNF-αand MMP-3 levels,the joint symptom score and the walking time of 20 m were compared between the two groups before and after treatment.Results The clinical efficacy of the treatment group was better than that of the control group(P<0.05).The serum levels of TNF-αand MMP-3 after treatment were significantly lower than before treatment(P<0.05),but there was no significant difference between the two groups(P>0.05).The scores of joint pain,joint swelling,joint dysfunction and joint tenderness after treatment in the two groups were lower than those before treatment(P<0.05),and the 20-minute walking time was decreased(P<0.05),and the difference of the two groups before and after treatment were also statistically significant(P<0.05),and the treatment group was larger than the control group.Conclusion The umbilical needle combined with far infrared physiotherapy in treating KOA can effectively inhibit the exudation of inflammatory cytokines,promote cartilage synthesis and repair,effectively relieve joint pain,improve joint activity and function,and delay the progression of the disease.
