厚朴排气合剂对瘢痕子宫产妇二次剖宫产后胃肠功能及泌乳的影响

摘　　要：目的观察厚朴排气合剂对瘢痕子宫产妇二次剖宫产后胃肠功能及泌乳的影响。方法将98例瘢痕子宫产妇二次剖宫产后患者随机分为2组。对照组39例进行术后常规治疗及护理,治疗组59例加服厚朴排气合剂。比较2组首次肠鸣音、排气、排便时间,检测血清胃肠激素血清胃泌素(GAS)、胃动素(MTL)及血管活性相关肠肽(VIP)水平,记录术后72 h胃肠道并发症情况,比较2组胃肠功能恢复优良率及泌乳情况(泌乳始动时间、72 h泌乳充足率),统计2组住院情况。结果治疗组术后肠鸣音恢复时间、排气时间及排便时间均短于对照组(P<0.05),治疗组术后3 d GAS、MTL水平较术后1 d升高,VIP水平降低(P<0.05),对照组各项激素水平无明显变化(P>0.05);治疗组术后3 d GAS、MTL水平均高于对照组,VIP水平低于对照组(P<0.05)。治疗组术后72 h腹胀及并发症总发生率低于对照组(P<0.05)。治疗组术后胃肠功能恢复优良率62.71%,高于对照组(46.15%,P<0.05)。治疗组泌乳始动时间早于对照组,72 h泌乳充足率高于对照组(P<0.05)。治疗组下床活动时间早于对照组,腹胀持续时间、禁食水时间、住院时间均短于对照组(P<0.05)。结论厚朴排气合剂能够调节瘢痕子宫产妇二次剖宫产后胃肠激素水平,促进胃肠功能的恢复及泌乳功能,利于产妇的整体康复。Objective To observe clinical effects ofHoupo Paiqi mixture on gastrointestinal function and lactation after scar uterus maternal secondary cesarean section.Methods 98 patients with scar uterus maternal secondary cesarean section were randomly divided into two groups.39 cases in control group were treated by routine treatment and nursing,59 cases in treatment group were treated by Houpo Paiqi mixture on the basis of control group treatment.The time offirst bowel sound,exhaust and defecation in two groups were compared.The levels of serum gastro-intestinal hormone gastrin(GAS),motilin(MTL)and vasoactive intestinal peptide(VIP)were measured,and 72 h after operation was recorded.The recovery rates of gastrointestinal function in the two groups were compared and the lactation status in the two groups(lactation start time and adequacy of lactation at 72 hours)were compared.The hospitalization status of the two groups were counted.Results The first bowel sound recovery time,venting time and defecation time after operation in treatment group were shorter than those in control group(P<0.05).The GAS and MTL levels 3 d after operation in treatment group were higher than those in 1 d after operation,and the VIP level was decreased(P<0.05).There was no significant change in the hormones in control group(P<0.05).The GAS and MTL levels 3 d after operation in treatment group were higher than those in control group,and the VIP level was lower in treatment group than in control group(P<0.05).The total incidence of abdominal distension and complications in treatment group at 72 h after operation was lower than that in control group(P<0.05).The excellent rate of recovery of gastrointestinal function after treatment was 62.71%in treatment group,which was higher than that in control group(46.15%,P<0.05).The initiation time of lactation in treatment group was earlier than that of the control group,and the adequacy of lactation at 72 h was higher than that of the control group(P<0.05).The time of ambulation in treatment group was earli
