充血性心衰患者血清肌钙蛋白、心肌酶谱检测的临床价值

摘　　要：目的探索充血性心衰患者血清肌钙蛋白、心肌酶谱水平检测的临床价值。方法选择2015年7月11日至2016年7月11日我院60例健康体检者(对照组)与60例疑似充血性心衰患者(观察组)为研究对象,对所有受检者进行血清肌钙蛋白、心肌酶谱水平检测,并分析此类指标的价值。结果观察组cTnI为(0.98±0.58)μg/L,LDH为(245.36±15.48)U/L,CK-MB为(29.58±1.98)U/L,CK为(265.65±5.47)U/L,均高于对照组的(0.11±0.02)μg/L、(124.63±14.05)U/L、(8.55±1.36)U/L、(142.81±4.54)U/L(P<0.05);观察组中,心功能轻、中、重衰竭患者的LDH、CK-MB、CK及cTnI水平比较,差异显著(P<0.05)。与单纯使用血清肌钙蛋白或心肌酶谱诊断比较,联合诊断的灵敏度、特异度均较高,漏诊率、误诊率均较低(P<0.05)。结论血清肌钙蛋白、心肌酶谱水平检测充血性心衰患者的效果显著,临床应用价值高。Objective To explore the detection value of serum troponin and myocardial enzyme profile in patients with congestive heart failure.Methods Sixty cases of healthy subjects with physical examination(enrolled in the control group)and 60 cases of patients with suspected congestive heart failure(enrolled in the observation group)in our hospital from July 11,2015 to July 11,2016 were selected in this study.All the subjects were given the serum troponin,myocardial enzyme profile detection.The values of such indicators were analyzed.Results The levels of cTnI,LDH,CK-MB and CK were respectively(0.98±0.58)Ug/L,(245.36±15.48),(29.58±1.98)and(265.65±5.47)U/L intheobservationgroup,w hichw ere higherthan(0.11±0.02)Ug/L,(124.63±14.05)U/L,(8.55±1.36)U/L a n d(142.81±4.54)U/L in the control group(P<0.05).In the observation group,the levels of LDH,CK-MB,CK and cTnI in patients with mild,moderate and severe heart failure were significantly different(P<0.05).Compared with the simple diagnosis of serum troponin or myocardial enzyme profile,the sensitivity and specificity of combined diagnosis were higher,miss diagnosis rate and misdiagnosis rate were lower(P<0.05).Conclusion Serum troponin and myocardial enzyme profile detection in the diagnosis of congestive heart failure is effective,which has high clinical value.
