陕西省铜川地区妇女人群HPV感染现状及其常见亚型流行特征

摘　　要：目的对陕西省铜川地区妇女人群人乳头状瘤病毒(human papillomavirus,HPV)感染及亚型分布特征进行研究。方法采用PCR反向点杂交技术对4115例宫颈脱落细胞标本进行HPV基因分型检测。结果 4115例妇女人群共检出HPV阳性者667例,总感染阳性率为16.21%,其中高危亚型(含多重高危)感染率为13.66%,低危亚型(含多重低危)感染率为1.56%,高低危亚型混合感染率为1.00%。单一亚型感染率为12.88%,多重亚型感染率为3.33%;在多重感染中以二重感染最为常见,达2.72%,占多重感染的81.75%。单一感染常见亚型依次为16、52、58、68、53和43型;多重感染常见亚型依次为16、58、52、53、66和56型,总体感染常见亚型依次为16、52、58、53、66和43型。≤20岁组妇女人群HPV感染率为13.73%(14/102),21~30岁组为13.62%(73/536),31~40岁组为14.73%(143/971),41~50岁组为16.30%(269/1650),51~60岁和>60岁组分别为18.99%(135/711)和22.76%(33/145),各组比较均有显著差异(P<0.05)。≤20岁、21~30岁、31~40岁、41~50岁、51~60岁和>60岁各年龄段感染常见亚型分别为16、52、58、53、66、56型,16、58、33、52、51、53型,16、52、58、68、53、66型,16、52、58、53、56、42型,16、58、52、66、43、53型,16、52、31、51、18、53型。结论陕西省铜川地区妇女人群宫颈HPV感染率随年龄增长而升高,具备特有的HPV亚型分布特征,各年龄段感染常见亚型亦有一定差异,16亚型感染最为常见。Objective To investigate human papillomavirus(HPV)infections and gene typing in women in Tongchuan in Shaanxi province.Methods 4115 cases of cervical cell specimens and 23 HPV genotypes were analyzed by PCR and reverse dot blot hybridization technique.Results The total infection rate was 16.21%.The rates of high-risk,low-risk and h-l-risk HPV infections were 13.66%,1.56%and 1.00%respectively.The infection rates of single and multiple subtypes were 12.88%and 3.33%respectively.Double infection was the most common(2.72%,112/4115)in cases of multiple infections,accounting for 16.79%(112/667)of the total cases of infections and 81.75%(112/137)of cases of multiple infections.The most common subtypes of total,single and multiple HPV infections were 16/52/58/53/66/43,16/52/58/68/53/43 and 16/58/52/53/66/56.The rates of HPV infections in≤20,21~30,31~40,41~50,51~60 and>60 age groups were 13.73%(14/102),13.62%(73/536),14.73%(143/971),16.30%(269/1650),18.99%(135/711)and 22.76%(33/145)respectively.The difference was of statistical significance(P>0.05).The common subtypes of≤20,21~30,31~40,41~50,51~60 and>60 age groups were 16/52/58/53/66/56,16/58/33/52/51/53,16/52/58/68/53/66,16/52/58/53/56/42,16/58/52/66/43/53,16/52/31/51/18/53 types.Conclusion The HPV infection rates of women in Tongchuan in Shaanxi province are gradually increased along with age and are typical of HPV subtype distribution involving different age groups.HPV 16 is the most common subtype.
