摘 要:目的探究初诊2型糖尿病(T2DM)患者甲状腺激素水平与糖代谢水平的关系及其临床价值。方法选取2014年6月-2016年12月我院内分泌科收治的238例T2DM患者,测定所有患者甲状腺激素水平和糖代谢水平,采用Spearman相关分析其相关性。结果不同性别在甲状腺激素水平、糖代谢指标及血脂指标方面差异不具有统计学意义(P>0.05)。随着FPG升高,TSH、T3等激素水平降低,差异具有统计学意义(P<0.05)。随着TSH水平升高,FPG、2h PG、Hb A1c、Ho MA-IR、TC和TG差异具有统计学意义(P<0.05)。FT3与BMI、FPG、2h PG、Hb A1c呈负相关(r=-0.153、-0.122、-0.321、-0.161,P<0.05),FT3、FT4和TSH与Ho MA-IR呈正相关(r=-0.561、0.601和0.246,P<0.05),TSH与Hb A1c呈负相关(r=-0.122,P<0.05),TT3和TT4与BMI、FPG、2h PG、Hb A1c、Ho MA-IR、TC和TG无相关性(P>0.05)。结论初诊2型糖尿病患者甲状腺激素水平对患者糖代谢水平评估具有重要意义。Objective To explore the relationships between thyroid hormone and glucose metabolism in patients with newly diagnosed type 2 diabetes mellitus(T2DM)and the clinical value.Methods 238 patients with T2DM who were treated in the Department of Endocrinology in our hospital between June 2014 and December 2016 were enrolled in this study.The levels of thyroid hormone and glucose metabolism were measured,and the correlations were analyzed by Spearman correlation analysis.Results The levels of thyroid hormone,glucose metabolism and serum lipids were not significantly different between the two genders(P>0.05).With the increase of FPG and the decrease of TSH and T3 levels,the difference was of statistical significance(P<0.05).With the increase of TSH levels,there was significant difference between FPG,2h PG,HbA1c,HoMA-IR,TC and TG(P<0.05).FT3 was negatively correlated with BMI,FPG,2hPG and HbA1c(r=-0.153,-0.122,-0.321,-0.161,P<0.05).FT3 and FT4 were positively correlated with TSH and HoMA-IR(r=-0.561,0.601 and 0.246,P<0.05)while TSH was negatively correlated with HbA1c(r=-0.122,P<0.05).There was no correlation between TT3,TT4 and BMI FPG,2hPG,HbA1c,HoMA-IR,TC and TG(P>0.05).Conclusion The thyroid hormone level is of great significance for evaluating the glucose metabolism level in patients with newly diagnosed type 2 diabetes mellitus.
