白细胞分离术治疗高白细胞白血病的效果观察及影响因素分析

摘　　要：[目的]探讨白细胞分离术在高白细胞白血病中的临床效果及其效果的影响因素,为高白细胞白血病的治疗提供参考。[方法]2014年1月—2017年4月收治的76例高白细胞性白血病病人进行137次白细胞分离术,比较分离前后血常规变化情况,包括白细胞(WBC)计数、血红蛋白(HGB)含量、血小板(PLT)计数等,采用多元逐步线性回归分析白细胞分离术的影响因素。[结果]白细胞分离术后血常规白细胞计数由(255.5±154.1)×109/L下降至(204.8±147.9)×109/L,血红蛋白含量由(70.0±29.4)g/L下降至(64.7±26.5)g/L,血小板由(167.1±245.3)×109/L下降至(116.2±152.6)×109/L,差异均有统计学意义(P<0.05)。分离前红细胞压积、水化、是否联合使用羟基脲及白细胞收集量、运行时间、单采次数均对白细胞分离术的效果有影响。[结论]白细胞分离术能快速去除白细胞,为治疗赢得时间,但其影响因素较多,临床实践中应积极规避,从而提升分离效果。Objective:To explore the clinical effect of leukocyte separation in high leukocyte leukaemia and its influencing factors,and to provide references for the treatment of high leukocyte leukemia.Methods:76 patients with hyper leukocytic leukemia patients in January 2014 to April 2017 in our hospital were selected for the 137 Leukocyte Reduction Procedures,compare the changes before and after the separation of white blood cell(WBC)count,hemoglobin concentration and platelet count.Multivariate stepwise regression analysis was used to analyze the influencing factors of leukocyte separation.Results:Compared the blood leukocyte separation before and after Leukocyte Reduction Procedures,WBC count decreased from(255.5±154.1)×10 9/L to(204.8±147.9)×10 9/L,HGB decreased from(70.0±29.4)g/L to(64.7±26.5)g/L,PLT count decreased from(167.1±245.3)×10 9/L to(116.2±152.6)×10 9/L.The differences were statistically significant(P<0.05).Before the separation of hematocrit,hydration,whether combined with hydroxyurea and WBC collection,operation period,the number of WBC apheresis influenced Leukocyte Reduction Procedures effects.Conclusion:Leukocyte separation could rapidly remove WBC and gain time for treatment.However,due to there were many influencing factors,it should be avoided actively in clinical practice,so as to enhance the separation effect.
