疏风通络法联合西药治疗支气管哮喘疗效及对血清炎症因子的影响

摘　　要：目的观察疏风通络法联合舒利迭(丙酸氟替卡松/沙美特罗)治疗支气管哮喘热哮患者的临床疗效及对血清白细胞介素-8(IL-8)、白细胞介素-4(IL-4)、干扰素-γ(IFN-γ)水平的影响。方法将60例支气管哮喘热哮患者随机分为对照组和研究组,对照组30例给予常规抗炎、吸氧、化痰、止咳、平喘等治疗,并给予沙美特罗替卡松粉吸入剂(舒利迭)吸入,l吸/次,2次/d;研究组30例在对照组治疗基础上加服由疏风解痉、宣肺通络中药组成的复方,1剂/d,2组疗程为14 d。随访14 d,观察2组临床疗效及治疗前后FVC、FEV_1、FEV_1/FVC和血清IL-8、IL-4、IFN-γ水平变化情况。结果治疗结束后,研究组临床总有效率显著高于对照组(P<0.05)。治疗后2组FVC、FEV_1、FEV_1/FVC及血清IL-8、IL-4、IFN-γ水平均显著改善(P均<0.05),且研究组改善程度显著优于对照组(P均<0.05)。结论疏风通络法联合舒利迭治疗支气管哮喘的疗效确切,并可降低血清IL-8、IL-4及IFN-γ等炎症因子指标水平。Objective It is to observe the clinical efficacy of Shufeng Tongluo therapy combined with Seretide(fluticasone propionate/salmeterol)on bronchial asthma of heat type and its effect on serum interleukin-8(IL-8),IL-4 and interferon-γ(IFN-γ)levels.Methods 60 patients with bronchial asthma of heat type were randomly divided into control group and study group.The control group(n=30)was treated with conventional therapy such as anti-inflammatory,oxygen absorption,relieving phlegm and cough,asthma,in addition,Seretide was given by inhalation,one inhalation/time,2 times/d.The study group was treated with traditional Chinese medicine that could Shufeng Jiejing and Xuanfei Tongluo based on the control group,1 dose/d.The course of treatment for the two groups was 14 days.After 14 days of follow-up,the clinical efficacy and changes of FVC,FEV 1,FEV 1/FVC,and serum IL-8,IL-4 and IFN-γlevels in the two groups were observed.Results After treatment,the clinical total effective rate of the study group was significantly higher than that of the control group(P<0.05).After treatment,the levels of FVC,FEV 1,FEV 1/FVC,and serum IL-8,IL-4 and IFN-γlevels were improved significantly in both groups,and the improvements in the study group were significantly better than those in the control group(P<0.05).Conclusion Shufeng Tongluo therapy combined with Seretide has a definite curative effect on bronchial asthma,and can reduce the levels of serum inflammatory factors such as IL-8,IL-4 and IFN-γ.
