超声引导下负压引流术联合乳痈1号方治疗哺乳期乳腺脓肿疗效及对血清黏附因子-1、肿瘤坏死因子-α、白细胞介素-1β的影响

摘　　要：目的探讨超声引导下多方向负压引流术联合乳痈1号方治疗哺乳期乳腺脓肿的临床疗效及对血清黏附因子-1(ICAM-1)、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)的影响。方法将96例哺乳期乳腺脓肿患者随机分为2组,对照组48例给予超声引导下多方向负压引流术治疗,研究组48例在对照组治疗基础上联合乳痈1号方治疗。统计2组患者脓腔愈合时间、换药次数、乳瘘发生情况、治疗后继续哺乳例数,观察治疗前及治疗2周后VAS评分及ICAM-1、TNF-α、IL-1β的水平变化情况,评价临床疗效。结果治疗后,研究组的临床治疗总有效率及继续哺乳率均显著高于对照组(P均<0.05),切口愈合时间显著短于对照组(P<0.05),换药次数显著少于对照组(P<0.05),乳瘘发生率显著低于对照组(P<0.05);治疗后,2组VAS评分及血清ICAM-1、TNF-α、IL-1β水平均显著低于治疗前(P均<0.05),且研究组均显著低于对照组(P均<0.05)。结论超声引导下多方向负压引流术联合乳痈1号方治疗哺乳期乳腺脓肿效果显著,可能与明显降低血清ICAM-1、TNF-α、IL-1β水平有关。Objective It is to investigate the clinical efficacy of ultrasound-guided multi-direction negative pressure drainage combined with Ruyong No.1 decoction on breast abscess in lactation and its effects on serum adhesion molecules-1(ICAM-1),tumor necrosis factor-α(TNF-α),and interleukin-1β(IL-1β).Methods Ninety-six patients with breast abscess in lactation were randomly divided into two groups,the control group(n=48)was given ultrasound-guided multi-direction negative pressure drainage,the study group(n=48)was treated with Ruyong No.1 decoction based on the control group.The wound healing time,frequency of dressing change,occurrence of breast fistula,number of consecutive breast-feeding cases were recorded,the clinical efficacy was evaluated,and the changes of VAS scores and levels of serum ICAM-1,TNF-α,IL-1βbefore and after treatment were observed in both groups.Results After treatment in the study group,the total effective rate of clinical treatment and the rate of continuous breastfeeding were significantly higher,the wound healing time was significantly shorter,the number of dressing changes was significantly less,the incidence of breast fistula was significantly lower than those in the control group(P<0.05).After treatment,the VAS scores and the levels of serum ICAM-1,TNF-α,and IL-1βin the two groups were significantly lower than those before treatment(P<0.05),and the indexes above in the study group were significantly lower than those in the control group(P<0.05).Conclusion Ultrasound-guided multi-direction negative pressure drainage combined with Ruyong No.1 decoction can effectively treat for breast abscesses during lactation,which may be related to the significant decrease of serum ICAM-1,TNF-αand IL-1β.
