肝移植受者中性粒细胞与淋巴细胞的比值及血小板与淋巴细胞的比值变化趋势探讨

摘　　要：目的:研究中性粒细胞与淋巴细胞的比值及血小板与淋巴细胞的比值在肝移植患者外周血中的变化并探索其临床意义。方法:收集20例在医院接受肝移植手术患者并纳入肝移植组,选择同期年龄相近的20名健康体检者并纳入健康对照组。比较两组术前及术后1个月的中性粒细胞、淋巴细胞、中性粒细胞与淋巴细胞的比值(NLR)、血小板、血小板与淋巴细胞的比值(PLR),并比较术后不同时间点之间中性粒细胞与淋巴细胞的比值、血小板与淋巴细胞比值的变化,分析其相关性。结果:肝移植组术前NLR与健康对照组比较差异有统计学意义(t=2.35,P<0.05);术前与术后第1 d及术后1个月PLR相比较差异无统计学意义;术后1个月肝移植组的淋巴细胞、NLR及血小板数与健康对照组比较差异有统计学意义(t=-5.884,t=2.534,t=-2.299;P<0.05);肝移植组术前及术后各时间点PLR与健康对照组比较差异无统计学意义。结论:肝移植组术后出现的NLR变化可能与术后反应性中性粒细胞增多、术后感染、移植后特有的中性粒细胞增多现象或服用免疫抑制剂有关。NLR可作为肝移植的监测指标之一。Objective:To study the changes of neutrophil to lymphocyte ratio(NLR)and platelet to lymphocyte ratio(PLR)in the peripheral blood(PB)of liver transplantation recipient and explore its clinical value.Methods:20 patients who received liver transplantation were divided in liver transplantation group,and 20 health persons with the same age who come from physical examination were divided in control group.And the neutrophil,lymphocyte,NLR,platelet,and PLR of pre-operative and 1month post-operation between the two groups were compared,and the changes of NLR and PLR at differently postoperative time point were compared.And then the correlations of them were further analyzed.Results:There was a significant difference in preoperative NLR between liver transplantation group and health control group(t=2.35,P<0.05).And the difference of PLR among pre-operation,postoperative 1st d and 1 month post-operation was no significant.The differences of lymphocyte,NLR and platelet count of 1 month post-operation between liver transplantation group and health group were significant(t=-5.884,t=2.534,t=-2.299,P<0.05).And the difference of PLR at various time point of preoperation and post-operation between the two groups were not significant.Conclusions:The change of the postoperative NLR of liver transplantation group may be relative with some phenomenon include postoperative reactive increasing of neutrophil,postoperative infection,special increasing of neutrophil post-transplantation or taking immunosuppressor.And the NLR can be used as one of monitoring indicators of liver transplantation.
