肝素钠盐水和生理盐水用于小儿外周静脉留置针封管有效性的Meta分析

摘　　要：[目的]评价肝素钠盐水在小儿外周静脉留置针封管中的效果。[方法]计算机检索中国知网、维普资讯网、万方数据库,收集关于外周静脉留置针肝素钠盐水与生理盐水封管的随机对照试验(RCT),对纳入的研究进行质量评价,采用RevMan 5.1软件进行Meta分析。[结果]共纳入研究8项。Meta分析结果显示:肝素钠盐水与生理盐水封管在降低留置针堵管率方面差异有统计学意义(P<0.000 1),肝素钠盐水的堵管率低于生理盐水组。两者在静脉炎的发生率差异无统计学意义(P=0.93)。两者在平均留置时间上的差异无统计学意义(P=0.21)。[结论]小儿采用肝素钠盐水封管的堵管率低于生理盐水封管;两种封管方式在静脉炎及平均留置时间方面无统计学差异。纳入文献质量一般,本结果尚待高质量RCT证实。Objective:To evaluate the efficacy of the Heparin sodium saline on tube sealing of peripheral venous indwelling needle for children.Methods:The randomized controlled trials on the Heparin sodium saline and normal saline on tube sealing of peripheral venous indwelling needle were retrieved by Chinese CNKI,VIP Information Network,Wanfang database.They were evaluated on the quality of included literatures and analyzed by Meta analysis.Results:A total of 8 studies were included,with a total of 556 patients.Meta analysis showed that heparin sodium saline and normal saline tube sealing had a statistically significant difference in reducing the rate of indwelling needle blocking(P<0.000 1),the blocking rate of heparin sodium saline was lower than that of normal saline group.There was no statistically significant difference in the incidence of phlebitis(P=0.93).There was no statistically significant difference in average retention time between the two groups(P=0.21).Conclusions:The blockage rate of heparin sodium saline tube sealing in children was lower than that in normal saline group.There was no statistically significant difference in phlebitis and average retention time between the two methods.The quality of included literatures was general,and the result has yet to be confirmed by high quality of RCT.
