摘  要:目的:探讨泌尿微创技术应用中肾盂高压与血尿的相关性。方法:收治肾结石患者65例,行泌尿微创技术治疗。术前测定血肌酐(SCr)、血清电解质(Na^+、K^+、Cl-)和血尿素氮(BUN)3项指标,术中使用尿动力学仪器监测肾盂压力,高压组为术中肾盂压力30 cm H_2O,且累计时间>10 min的患者,共34例;其余31例患者为低压组。术后测定SCr、Na^+、K^+、Cl^-、BUN。结果:术后两组患者血Na^+、Cl-、SCr、BUN均升高,血K^+降低;高压组变化更大,两组比较,差异有统计学意义(P<0.05)。术后第1、2、3、7天检测尿常规比较,高压组阳性率与低压组比较,差异有统计学意义(P<0.05)。结论:肾结石患者应用泌尿微创技术,术中严格控制肾盂压力,避免肾盂压力持续过高损伤患者早期的肾功能,保持正常的肾盂压力,对促进患者康复具有重要价值。Objective:To explore the correlation between renal pelvis pressure and hematuria during the application of minimally invasive urology.Methods:65 cases of renal calculi patients treated with minimally invasive urology were selected.3 indexes of serum creatinine(SCr),serum electrolyte(Na+、K+、Cl-)and blood urea nitrogen(BUN)were measured before operation.Urodynamic instruments were used to monitor the pressure of the renal pelvis during the operation.In the high-pressure group,the intra-renal pelvis pressure was 30 cmH2O and the cumulative time exceeded 10 minutes,a total of 34 patients,the remaining 31 patients were treated as the low pressure group.After operation,SCr、Na+、K+、Cl-and BUN were measured.Results:After the operation,the blood Na+,Cl-,SCr and BUN increased in the two groups,and the blood K+decreased.The change of the high pressure group was greater,and the difference between the two groups was statistically significant(P<0.05).On the first,second,third and seventh day after the operation,the urine routine was compared.The positive rate of the high pressure group was compared with the low pressure group,and the difference was statistically significant(P<0.05).Conclusions:Urinary minimally invasive technique is used in patients with kidney stones.Intraoperative strict control of renal pelvis pressure to avoid early renal function in patients with sustained renal pelvis pressure injury and maintain normal pelvis pressure is of great value in promoting patient rehabilitation.
