拉玛泽呼吸法结合无创自由体位分娩法在产科的运用体会

摘　　要：目的:探讨拉玛泽呼吸法结合无创自由体位分娩法在产科临床应用价值。方法:回顾性分析2016年1-12月于本院阴道试产的初产产妇120例,根据分娩护理方式不同分成两组,每组各60例。对照组采取常规体位分娩,观察组给予拉玛泽呼吸法结合无创自由体位分娩。观察两组产妇产时疼痛情况、分娩控制感、产程时间及新生儿Apgar评分。结果:观察组产妇VAS评分明显低于对照组(P<0.05);观察组产妇分娩控制感明显优于对照组(P<0.05);观察组产妇第一产程、第二产程及总产程时间均较对照组明显缩短(P<0.05);第三产程时间与对照组比较,差异无统计学意义(P>0.05);观察组产妇自然分娩率为93.33%,明显高于对照组80.00%(P<0.05);观察组产妇剖宫产率与引导助产率分别为6.67%、0,明显低于对照组18.33%、1.67%(P<0.05);两组产妇分娩后,均未出现重度窒息新生儿,观察组新生儿Apgar评分≥8分新生儿明显多于对照组(P<0.05);4~7分新生儿明显少于对照组(P<0.05)。结论:全程陪伴分娩的过程中实施拉玛泽呼吸法结合无创自由体位分娩法能缓解产妇分娩过程中的疼痛,促进自然分娩快速、高效完成,降低剖宫产率,安全性高,值得临床推广。Objective:To investigate the Ramaze breathing method combined with noninvasive free posture delivery method in obstetric clinical application.Method:120 cases of maternal vaginal delivery retrospective analysis in our hospital were selected from January to December 2016,according to the different nursing methods they were divided into two groups,60 cases in each group.The control group was given conventional position childbirth,the observation group was given Ramaze breathing method combined with noninvasive free posture delivery.The maternal pain,birth control,birth process and neonatal Apgar score of two groups were observed.Result:The VAS score of the observation group was significantly lower than that of the control group(P<0.05);the maternal birth control in observation group was significantly better than that of the control group(P<0.05);the first stage of labor,the second stage of labor and the total labor time in observation group were significantly shorter than those of the control group(P<0.05);the third stage of labor time of observation group compared with the control group,the difference was not statistically significant(P>0.05);the natural birth rate in observation group was 93.33%,higher than the control group of 80.00%(P<0.05);of the cesarean section rate and guide the delivery rate of observation group were respectively 6.67%and 0,significantly lower than those of the control group 18.33%and 1.67%(P<0.05);there were no severe neonatal asphyxia of two groups after childbirth,the neonatal Apgar score more than 8 points neonates in observation group were higher than that of control group(P<0.05);4-7 score neonates in observation group were significantly less than the control group(P<0.05).Conclusion:The method of noninvasive free position delivery method can alleviate the pain during childbirth Ramaze breathing accompanied delivery in the implementation process,promote natural delivery fast,efficiently,reduce the cesarean section rate,high safety,it is worthy of promotion.
