摘　　要：目的:探讨封闭负压创面治疗技术联合薄型游离皮瓣修复手部热压伤的临床治疗可行性研究。方法:选取2016年2月-2017年1月本院收治手部热压伤患者46例,均给予坏死组织清创术、深部组织损伤探查、薄型游离皮瓣修复术及封闭负压创面治疗技术,并跟踪治疗结果。结果:本组46例均顺利完成手术,无一例发生感染并发症,45例皮瓣完全成活,创面均Ⅰ期愈合,皮瓣质地、外观与周围正常皮肤相似。1例足背皮瓣缺血性坏死,给予植皮修复、定期换药及敏感抗生素治疗,获得愈合。术后随访6个月,手功能恢复情况:优21例,良17例,差8例,劣0例,优良率为82.60%。结论:封闭负压创面治疗技术联合薄型游离皮瓣修复手部热压伤具有较高的皮瓣成活率,可改善患者预后,值得临床推广应用。Objective:To discuss the clinical feasibility study of negative pressure creation treatment technique combined with skin thin flap repair to treat with burn and compression of radical.Method:46 patients with burn and compression of radical were selected from February 2016 to January 2017 in our hospital,All they received debridement of necrotic tissue,deep tissue damage detection,skin thin flap repair and negative pressure creation treatment technique,treatment results were followed.Result:The 46 cases were successfully completed surgery,no case of infection complications occurred,45 cases flap survived completely,all of them wereⅠhealing of the wound,the flap were similar to the surrounding normal skin texture and appearance.1 case of dorsalis pedis ischemic necrosis flap was given skin graft to repair the bandage,regular and sensitive antibiotics for healing.After follow-up for 6 months,the function of hand function recovered good of 21 cases,inferior of 17 cases,poor of 8 cases,fine rate was 82.60%.Conclusion:Negative pressure creation treatment technique combined with skin thin flap repair to treat with burn and compression of radical has higher flap survival rate,can improve the prognosis of patients,worthy of clinical popularization and application.
