环形石膏折顶整复固定治疗第五掌骨拳击骨折的效果及对高凝状态的影响

摘　　要：目的:探讨环形石膏折顶整复固定治疗第五掌骨拳击骨折的效果及对血清超敏C反应蛋白(hs-CRP)、D-二聚体(D-D)的影响。方法:将2015年9月-2017年6月因第五掌骨拳击骨折就诊的60例患者纳入研究并随机分组。对照组30例患者采用传统手法整复铝板固定治疗,治疗组30例则采用环形石膏折顶整复固定治疗,比较两组的临床疗效。结果:治疗后,治疗组hs-CRP、D-D水平均低于对照组,差异均有统计学意义(P<0.05);治疗组的并发症发生率低于对照组,一次性解剖复位率高于对照组,差异均有统计学意义(P<0.05);治疗组优良率为93.33%(28/30),高于对照组的70.00%(21/30),差异有统计学意义(P<0.05)。结论:环形石膏折顶整复固定治疗第五掌骨拳击骨折效果显著,可促进骨折修复,安全可靠,值得推广。Objective:To investigate the effect of ring-shaped plaster-fold fixation on fifth metacarpal fracture and its effect on serum high-sensitivity C-reactive protein(hs-CRP)and D-dimer(D-D).Method:Sixty patients from September 2015 to June 2017 were included in the study and randomized by fifth metacarpal fracture.In the control group,30 patients were treated with traditional method of aluminum plate fixation,30 patients in the treatment group were treated with annular plastering and fixed treatment,the clinical efficacy between the two groups were compared.Result:After treatment,the levels of hs-CRP and D-D in the treatment group were significantly lower than those in the control group,the differences were statistically significant(P<0.05);the incidence of complication in the treatment group was lower than that in the control group,and the reduction rate was higher than that in the control group,the differences were statistically significant(P<0.05);the excellent and good rate was 93.33%(28/30)in the treatment group,which was higher than 70.00%(21/30)in the control group,the difference was statistically significant(P<0.05).Conclusion:The treatment of the fifth metacarpal bone fractures with annular plastering is very effective,which can promote fracture repair,safe and reliable,and is worth popularizing.
