摘 要:目的探讨足踝部定量活动仪辅助踝泵运动对骨科患者术后疼痛及下肢静脉血回流的影响。方法选取2016年7月—2017年3月在深圳市某三级甲等医院接受脊柱、骨盆、髋关节及下肢手术的患者68例,采用随机数字表法分为对照组与观察组。对照组患者术后予以骨科常规护理及踝泵运动,观察组患者在对照组基础上使用足踝部定量活动仪辅助进行踝泵运动,比较2组患者手术当天麻醉清醒后、术后第3天及术后第7天运动后疼痛评分、股静脉血流速度及峰速度。结果骨科术后患者运动后疼痛评分,组间比较差异有统计学意义(F=12.752,P<0.001),观察组疼痛评分低于对照组。不同时间点骨科术后患者运动后疼痛评分比较,差异有统计学意义(F=182.533,P<0.001)。组间与时间无交互作用(F=0.073,P=0.436)。骨科术后患者运动后股静脉血流速度组间比较差异有统计学意义(F=13.327,P<0.001),观察组股静脉血流速度高于对照组。不同时间点骨科术后患者运动后股静脉血流速度比较,差异有统计学意义(F=193.759,P<0.001)。组间与时间无交互作用(F=0.043,P=0.958)。骨科术后患者运动后股静脉血流峰速度组间比较差异有统计学意义(F=12.569,P<0.001),观察组股静脉血流峰速度高于对照组。不同时间点骨科术后患者运动后股静脉血流峰速度比较,差异有统计学意义(F=161.458,P<0.001)。组间与时间无交互作用(F=0.048,P=0.277)。结论足踝部定量活动仪辅助骨科术后患者进行踝泵运动,能减轻患者运动后疼痛,加快股静脉血回流速度。Objective To investigate the effectiveness of ankle pump movement aided by ankle quantitative activity instrument on the pain and venous backflow of lower extremities after orthopedics surgery.Methods A total of 68 patients with spine,pelvis,hip and lower extremity surgery from July 2016 to March 2017 in one ClassⅢGradeⅠhospital from Shenzhen were selected and randomly divided into control group and observation group.The control group received routine orthopedic nursing and ankle pump movement and the observation group routine orthopedic nursing and ankle pump movement aided by ankle quantitative activity instrument.The pain score,venous backflow velocity and peak velocity of lower extremities were compared between the two groups after the operation on the day after anesthesia,third days after the operation and seventh days after the operation.Results The intergroup comparison of pain score,venous backflow velocity and peak velocity indicated statistical significance(F=12.752,P<0.001,F=13.327,P<0.001 and F=12.569,P<0.001 respectively).The pain score in the observation group was lower than that in the control group and that in different time points showed statistical significance(F=182.533,P<0.001)but no interaction was found in two groups and different time points(F=0.073,P=0.436).Venous backflow velocity of lower extremities of the observation group was faster than that of the control group and that in different time points showed statistical significance(F=193.759,P<0.001)but no interaction was found in two groups and different time points(F=0.043,P=0.958).Peak velocity of lower extremities of the observation group was higher than that of the control group and that in different time points showed statistical significance(F=161.458,P<0.001)but no interaction was found in two groups and different time points(F=0.048,P=0.277).Conclusion Ankle pump movement aided by ankle quantitative activity instrument is helpful for patients after orthopaedic surgery to alleviate the pain of patients after the exerci
