摘　　要：目的:探讨经支气管镜对局部病灶行支气管肺泡灌洗加全身抗感染治疗重症肺部感染患者的疗效。方法:选取本院2015年7月-2017年7月收治的48例重症肺部感染患者,根据治疗方式的不同分为对照组和观察组,每组各24例。对照组采取全身抗感染治疗,观察组在此基础上,采取经支气管镜对局部病灶行支气管肺泡灌洗治疗。观察对比两组治疗前后血气分析指标(SpO_2、PaCO_2、PaO_2)变化、住院时间、症状消失时间以及临床疗效。结果:治疗后观察组SpO_2、PaCO_2、PaO_2等指标与对照组相比改善显著,比较差异均有统计学意义(P<0.05)。观察组住院时间、症状消失时间等明显短于对照组,比较差异均有统计学意义(P<0.05)。观察组治疗有效率为95.83%(23/24)明显高于对照组的79.17%(19/24),差异有统计学意义(P<0.05)。结论:针对重症肺部感染患者采取经支气管镜对局部病灶行支气管肺泡灌洗加全身抗感染治疗的效果非常显著,既能显著改善患者各种症状,又能缩短其住院时间,加速其病情恢复,因此值得临床应用推广。Objective:To evaluate the efficacy of bronchoscopies for the treatment of patients with severe pulmonary infection by bronchoalveolar lavage plus systemic anti-infection.Method:48 patients with severe pulmonary infection admitted from July 2015 to July 2017 in our hospital were divided into control group and observation group according to the different treatment methods,24 cases in each group.The control group was treated with systemic anti-infective therapy,on this base,the observation group was treated with bronchoalveolar lavage through bronchoscopy.The changes of blood gas analysis indexes(SpO2,PaCO2,PaO2),length of hospital stay,the time of symptom disappearance and the clinical efficacy of two groups were observed and compared before and after treatment.Result:After treatment,SpO2,PaCO2,PaO2 and others in the observation group improved significantly compared with the control group,the difference was statistically significience(P<0.05).The observation group hospitalization time and other symptoms disappeared significantly shorter compared with the control group,the differences were statistically significience(P<0.05).The effective rate in observation group was 95.83%significantly higher than 79.17%in the control group,the difference was statistically significant(P<0.05).Conclusion:The treatment of severe pulmonary infection by bronchoscopy for local lesions with bronchoalveolar lavage plus systemic anti-infective therapy is very significant,both significantly improve the symptoms of patients,but also shorten their hospital stay and accelerate the recovery of their condition,it is worth to promote clinical application.
