腰椎间盘内注射结合超激光治疗椎间盘源性腰痛的远期疗效分析

摘　　要：目的:探讨腰椎间盘内注射结合超激光治疗椎间盘源性腰痛的远期疗效。方法:选择2015年1月-2016年12月在本院治疗的椎间盘源性腰痛患者80例为研究对象,按随机数字表法分为观察组与对照组,各40例。观察组采用腰椎间盘内注射激素类药物联合超激光的治疗方法,对照组采用针灸+牵引+红外线的治疗方法。比较两组治疗前后视觉模拟评分法(VAS)评分及Oswestry功能障碍指数(ODI),比较两组治疗满意度,随访12个月比较两组复发率。结果:治疗后3、6、12个月,两组VAS评分与ODI指数均显著低于治疗前,差异均有统计学意义(P<0.05);观察组治疗后3、6、12个月,VAS评分与ODI指数均显著低于对照组,差异均有统计学意义(P<0.05)。观察组总体治疗满意度高于对照组,差异有统计学意义(P<0.05)。两组患者随访12个月至随访结束,观察组复发率为7.5%,对照组复发率为25.0%,两组比较差异有统计学意义(P<0.05)。结论:腰椎间盘内注射结合超激光治疗椎间盘源性腰痛具有较好的远期疗效,值得临床推广。Objective:To discuss the long term effect of lumbar intervertebral disc injection combined with super laser in the treatment of lumbar discogenic back pain.Method:From January 2015 to December 2016,80 cases with lumbar discogenic back pain were selected as subjects,according to the random digital table method,they were divided into the observation group and the control group,40 cases in each group.The observation group was treated by injecting hormone drugs into lumbar intervertebral disc combined with super laser,and the control group was treated with acupuncture,traction and infrared ray.Visual analogue score(VAS)and Oswestry dysfunction index(ODI)before and after treatment,treatment satisfaction,and recurrence rate of two groups were compared.Result:VAS and ODI after 3,6 and 12 months of treatment in two groups were lower than those of before treatment(P<0.05).Treatment satisfaction in the observation group was higher than that in the control group(P<0.05).The recurrence rate in the observation group was 7.5%and the control group was 25.0%,there was significant difference between two groups(P<0.05).Conclusion:Lumbar intervertebral disc injection combined with super laser in the treatment of lumbar discogenic back pain has good long-term effect,and it is worthy of clinical promotion.
