摘　　要：目的:观察前列地尔注射液改善缺血皮瓣微循环的作用。方法:40只SD大鼠先行背部2×7cm皮瓣手术后平分为两组,分别尾静脉注射前列地尔注射液(0.8ml/kg,实验组)和等量生理盐水(对照组)。每日一次,持续7天。采用激光多普勒技术分别于术前、术后即刻(0min)、60min、1天、4天、7天测量各组大鼠皮瓣血流灌注量;并在术后第7天测量两组皮瓣成活面积;同时取两组大鼠皮瓣病理切片行HE染色观察皮瓣组织学和微血管口径变化,CD34染色计数微血管数量。结果:两组皮瓣近、中、远段区域在术前、术后0min、60min血流灌注量差异均无统计学意义(P>0.05)。术后第1天,实验组远段血流量高于对照组(P<0.05)。术后第4天和第7天实验组血流量近、中、远段亦明显高于对照组(P<0.05,P<0.01);且不同区段血流量与蒂部距离呈显著负相关(r_(60min)=-0.58,P<0.01;r_(1天)=-0.69,P<0.01;r_(4天)=-0.52,P<0.01;r_(7天)=-0.83,P<0.01)。与药物作用时间呈显著正相关(r_近=0.71,P<0.01;r_中=0.58,P<0.01;r_远=0.54,P<0.01)。实验组皮瓣成活面积大于对照组(12.12±0.89cm^2 vs 9.53±0.55cm^2,P<0.01)。HE染色显示实验组近、中区段皮瓣组织炎症改善和微血管扩张均较对照组明显。CD34染色显示实验组各区段微血管密度均高于对照组(P<0.05)。结论:前列地尔注射液可有效改善缺血皮瓣微循环。Objective:To observe of the effect of alprostadil on improvement blood supply in rats ischemia skin flap.Method:The rats were divided into 2 groups after the flap(2cm×7cm)was created on the back of the rats.Tail vein injection of alprostadil injection(0.8ml/kg,experimental group)and equal amount of normal saline(control group).The drug applied daily for 7 days.Blood flow perfusion in skin flap of both groups was monitored with laser doppler line scanner before surgery and after operation 0min,60min,1day,4day and 7day.On the 7th postoperative day,the surviving areas of the skin flaps were appraised.The microvascular diameter and flap histology were observed by HE staining,the number of microvessels were observed by CD34 staining.Results:There was no significant difference in the blood flow perfusion between the two groups in the near,middle and far segments in preoperative and postoperative 0min,60min(P>0.05).On the first day after operation,the blood flow volume of the distal flap in the experimental group was significantly higher than that in the control group.On the fourth and seventh days after operation,the blood flow volume in the proximal,middle and distal segments of the experimental group was also significantly higher than that in the control group(P<0.05 or P<0.01).The blood flow in different regions was negatively correlated with the distance of the pedicle(r 60min=-0.58,P<0.01;r 1天=-0.69,P<0.01;
