丙型肝炎核心抗原(HCV-cAg)在临床中的应用价值评价

摘　　要：目的:评价酶联免疫吸附试验(ELISA)检测丙型肝炎核心抗原(HCV-c Ag)在临床中的应用价值。方法:收集丙型肝炎抗体(HCV-Ab)阳性血清标本15例,HCV-Ab阴性血清标本170例,共计185例,同时对15例HCVAb阳性和170例HCV-Ab阴性的血清标本进行HCV-c Ag检测,同时使用RT-PCR进行HCV-RNA对照检测。结果:15例HCV-Ab阳性的标本中,HCV-c Ag阳性为5例;170例HCV-Ab阴性的标本中,HCV-c Ag阳性为2例,同时以HCV-RNA检测结果为对照,HCV-c Ag检测方法的敏感性和特异性分别为86%和98.9%,且与HCV-RNA检测方法比较,无统计学意义(P<0.05)。结论:HCV-c Ag检测方法的敏感性、特异性较好,可以作为HCV-Ab检测方法的补充,对丙型肝炎的早期诊断意义重大。Objective:To evaluate the clinical value of Hepatitis C virus core antigen(HCV-cAg)tested by ELISA assay.Methods:The serum samples of 15 cases of hepatitis C antibody-positive patients and 170 cases of hepatitis C antibody-negative patients in our hospital from January 2016 to December 2016 were collected.HCV core antigen was detected in 15 HCV-Ab positive and 170 HCV-Ab negative serum samples,and HCV-RNA was also tested simultaneously by fluorescent PCR assay for quantification.Results:Of the 15 HCV-Ab positive specimens,5 were positive for HCV-cAg,and of the 170 HCV-Ab negative specimens,2 were positive for HCV-cAg.The sensitivity and specificity of the HCV core antigen testing assay were 86%and 98.9%respectively compared with the results of HCV-RNA testing method,and there was no significant differences between HCV core antigen testing assay and HCV-RNA testing assay(P<0.05).Conclusion:HCV core antigen testing assay has satisfactory sensitivity and specificity,which could be used as a supplement to HCV-Ab detection assay and is of great importance in the early diagnosis of hepatitis C.
