闭合复位无头加压螺钉治疗中老年股骨颈骨折临床分析

摘　　要：目的:探讨无头加压螺钉治疗中老年股骨颈骨折的临床疗效。方法:47例中老年股骨颈骨折随机分为试验组和对照组,其中试验组24例予以C形臂透视下徒手闭合复位股骨颈骨折断端,复位理想后置入3枚无头加压螺钉固定骨折断端。对照组23例采用相同复位方法,但采用AO中空螺钉固定骨折断端。比较两组患者间内固定螺钉退出例数、骨折不愈合及股骨头坏死例数、平均愈合时间、总优良率的差异。结果:试验组并发症发生率较对照组低(P<0.05);试验组平均骨折愈合时间较对照组短(P<0.05);试验组髋关节Harris评分总优良率较对照组高,两组间差异有统计学意义(P<0.01)。结论:闭合复位无头加压螺钉治疗股骨颈骨折,能持续、有效的对骨折断端加压,降低了骨折不愈合、退钉及股骨头坏死发生率,术后髋关节功能恢复良好。Objective:To explore the curative effect of headless compression screw in the treatment of femoral neck fracture in the middle and old age.Methods:Fourty-seven cases of senile femoral neck fracture were randomly divided into experimental group and control group;24 cases in the experimental group were treated with C-arm fluoroscopy and closed reduction of femoral neck fracture,and 3 headless compression screws were inserted into the fracture end after being reset ideally.In the control group,23 cases were treated with the same reduction method,but AO hollow screw was used to fix the broken end.The differences in the number of exiting cases of internal fixation screws,nonunion of fracture and femoral head necrosis,the average healing time and total superior rate were compared between the two groups.Results:The incidence of complications in the experimental group was lower than that in the control group(P<0.05).The average fracture healing time of the experimental group was shorter than that in the control group(P<0.05).The total superior rate of the hip-joint Harris score in the experimental group was higher than that in the control group,and the difference between the two groups was statistically significant(P<0.01).Conclusion:The treatment of femoral neck fracture with closed reduction headless compression screw can be sustained and effective,which can reduce the incidence of nonunion,denailing and necrosis of femoral head,and recover the function of hip joint well after operation.
