摘 要:目的:探讨结核性胸膜炎患者血清及胸腔积液中INF-γ、IL-6和IL-17细胞因子水平变化及其在临床上的意义。方法:选取2012-08/2015-09石家庄市第五医院呼吸内科住院治疗的患者作为研究对象,其中结核性胸膜炎患者50例(A组),非结核性胸膜炎患者50例(B组),同期入院检查的正常健康人50例(C组)。采用双抗体夹心ELISA方法测定,Western Blot验证ELISA检测结果,比较并分析各组患者血清及胸腔积液中干扰素(INF-γ)、白介素IL-6和IL-17细胞因子水平变化。结果:A组、B组患者胸液中的INF-γ、IL-6和IL-17水平均明显高于血清中INF-γ、IL-6和IL-17水平,差异有统计学意义(P<0.05),且A组血清中INF-γ、IL-6和IL-17水平均明显高于B组、C组,B组与C组血清中INF-γ、IL-6和IL-17水平比较,差异无统计学意义(P>0.05),A组胸液中的INF-γ、IL-6和IL-17水平明显高于B组,差异有统计学意义(P<0.05)。结论:结核性胸膜炎患者中INF-γ、IL-6和IL-17水平存在异常表达,且胸腔积液中的INF-γ、IL-6和IL-17水平均高于血清,在诊断结核性胸膜炎中有重要意义,联合检测INF-γ、IL-6和IL-17对于结核性胸膜炎的诊断价值更高。Objective:To investigate the level of IL-6,IL-17 and INF-γin serum and pleural effusion of patients with tuberculosis pleurisy and its clinical significance.Methods:Patients admitted to Fifth Hospital of Shijiazhuang from August 2012 to September 2015 were selected as the research objects,including 50 cases with tuberculosis pleurisy(group A)and 50 cases with non-tuberculosis pleurisy(group B).50 normal healthy people were selected into group C.The levels of INF-γ,IL-6 and IL-17 in serum and pleural effusion were detected by double antibody sandwich ELISA method and compared.Results:The levels of INF-γ,IL-6 and IL-17 in pleural effusion in group A and group B were significantly higher than those of serum(P<0.05),and the levels of INF-γ,IL-6 and IL-17 in serum in group A were significantly higher than those of group B and C.There was no difference between group B and C about the levels of INF-γ,IL-17,IL-6 in serum(P>0.05).The levels of INF-γ,IL-6 and IL-17 in pleural effusion in pleural effusion in group A were significantly higher than those of group B(P<0.05).Conclusion:The levels of INF-γ,IL-6 and IL-17 in patients with tuberculosis pleurisy are abnormal.The levels of INF-γ,IL-6 and IL-17 in pleural effusion are higher than those of serum,which is important in the diagnosis of tuberculous pleurisy.The combined detection of INF-γ,IL-6 and IL-17 has higher value in the diagnosis of tuberculosis pleurisy.
