中药湿敷对糖尿病足溃疡患者血管介入术后疗效影响

摘　　要：目的观察中药湿敷干预糖尿病足溃疡脉络瘀热证血管介入术后的临床疗效。方法将57例糖尿病足溃疡患者随机分为治疗组和对照组,治疗组29例,对照组28例。2组均给予下肢动脉血管介入术及基础治疗,治疗组在对照组基础上给予中药湿敷创面,治疗8周后观察临床疗效。结果治疗组、对照组临床总有效率分别为93.10%和78.57%,治疗组优于对照组(P<0.05)。经血管介入治疗,2组踝肱比(ABI)水平均比治疗前明显减小(P<0.01),组间比较,P>0.05。治疗后2组中医证候积分均有改善(P<0.01),治疗组明显优于对照组(P<0.01)。2组创面面积、伤腔容积均比治疗前明显减小(P<0.01),治疗组改善优于对照组(P<0.05);治疗后2组创面愈合率组间比较,治疗组优于对照组(P<0.05)。结论糖尿病足溃疡患者经血管介入治疗后,血供状况得以改善,联合中药湿敷能进一步促进创面愈合,优于单纯接受血管介入治疗。OBJECTIVE To observe the clinical effect of Chinese medicine wet compress in the intervention of diabetic foot ulcers of meridians and collaterals heat stasis accumulation type after interventional vascular surgery.METHODS 57 patients with diabetic foot ulcers were randomly divided into the treatment group(29 cases)and the control group(28 cases).Both groups were given arterial interventional therapy of lower extremity and the basic treatment.On the basis of the control group,the treatment group was also given Chinese medicine wet compress treatment.The clinical effect was observed after 8 weeks of treatment.RESULTS The total effective rate of the treatment group and the control group was 93.10%and 78.57%respectively,the treatment group was better than the control group(P<0.05).After the interventional vascular surgery,the ankle brachial indexes(ABI)in both groups were significantly decreased than those of the pre treatment(P<0.01),and P>0.05 compared between groups.TCM syndrome scores in both groups were improved after the treatment(P<0.01),and the treatment group was improved more significant than the control group(P<0.01).The wound area and wound volume of the two groups were significantly decreased than those of the pre treatment(P<0.01),the treatment group was improved more significant than the control group(P<0.05).After the treatment,the wound healing rate of the treatment group was better than that of the control group compared between two groups(P<0.05).CONCLUSIONS After the interventional vascular surgery,blood supply state of diabetic foot ulcers patients is improved,but the wound can be further healed through combining Chinese medicine wet compress.The effect of the combination treatment is better than interventional vascular surgery only.
