摘 要:目的探讨体脂分布对冠心病患者的预测价值。方法序贯选择2015年9月到2016年12月在右江民族医学院附属医院住院并行冠脉造影(CAG)确诊为冠心病的患者153例,分为单支冠脉病变组65例、双支冠脉病变组41例及多支冠脉病变组47例,同时选取健康成人40名作为对照(正常组)。用TANITAs MC-180测定人体内脏脂肪含量(VFC)和皮下脂肪含量(SFC),同时测量身高、体重、腰围(WC)、臀围(HC),计算体重指数(BMI),明确患者冠脉病变支数和冠脉Gensini积分情况,比较各组体脂成分与冠脉病变的相关性。结果多支冠脉病变患者SFC与正常组比较,差异有统计学意义(P<0.05);VFC与冠脉病变支数及Gensini积分呈正相关(P<0.05);SFC与低密度脂蛋白胆固醇(LDL-C)、冠脉病变支数及Gensini积分呈正相关(P<0.05)。结论内脏脂肪含量和皮下脂肪含量与冠脉病变支数及Gensini积分密切相关,可作为冠心病风险评估的重要指标之一。Objective To discuss the predictive value of body fat distribution in patients with coronary heart disease.Methods 153 cases who were diagnosed with coronary heart disease by coronary arteriography(CAG)at Affiliated Hospital of Youjiang Medical University for Nationalities from September,2015 to December,2016 were sequentially selected.They were divided into single vessel lesion group(65 cases),double vessel lesions group(41 cases)and multivessel lesions group(47 cases).At the same time,40 healthy adults were selected as control group.Then,visceral fat content(VFC)and subcutaneous fat content(SFC)were measured by TANITAsMC-180,and height,weight,waist circumference(WC)and hip circumference(HC)were measured.The body mass index(BMI)was calculated,and the number of coronary artery lesions and the score of coronary Gensini were assured.Correlation between body fat and coronary artery disease in each group was compared.Results The difference of SFC in the multivessel lesions group was statistically significant while comparing with that in the control group(P<0.05).There was a positive correlation between VFC and the number of coronary coronary lesions vessels and Gensini score(P<0.05).In addition,VFC was positively correlated with low-density lipoprotein cholesterol(LDL-C),coronary artery lesions vessels and Gensini score(P<0.05).Conclusion VFC and SFC are positively correlated with coronary artery lesions vessels and Gensini score,which can be used as one of the important indicators for the risk assessment of coronary heart disease.
