摘 要:目的探讨B超引导联合改良赛丁格技术(MST)运用对肿瘤患者外周静脉置入中心静脉导管(PICC)致上肢静脉血栓形成的影响,为临床预防血栓发生提供依据。方法将2015年6月至2017年1月需要进行PICC置管的216例患者作为研究对象。按照穿刺方式及穿刺部位进行分组,采用B超引导联合MST在肘关节上3~10 cm进行穿刺置管者设为观察组(119例);采用盲穿技术在肘关节以下2~3 cm进行置管者设为对照组(97例)。两组患者置管后采用常规护理方式护理,观察两组患者静脉血栓的发生率。结果观察组发生静脉血栓3例(2.52%),对照组发生静脉血栓9例(9.28%),两组发生率比较差异有统计学意义(P<0.05)。结论 B超引导联合改良赛丁格技术于肘上置管能使肿瘤患者PICC致血栓形成的发生率降低,延长导管使用时间,降低非计划性拔管率,提高患者带管安全性。Objective To investigate the effect of B-ultrasound guidance combined with modified Seldinger’s technique(MST)on upper extremity vein thrombosis induced by peripherally inserted central venous catheters(PICC),so as to provide evidence for the prevention of thrombosis in clinical practice.Methods 216 patients who needed PICC catheterization from June,2015 to January,2017 were selected as research subjects.Grouping was performed according to the way and location of the puncture.Patients who were given puncture catheterization by B-ultrasound guidance combined with MST at 3~10 cm above elbow joint were selected as observation group(119 cases),and patients who were given catheterization by blind puncture at 2~3 cm below elbow joint were selected as control group(97 cases).Both groups were given routine nursing care after catheterization,and the incidence of venous thrombosis was observed.Results There were 3 cases of venous thrombosis(2.52%)in the observation group and 9 cases(9.28%)of venous thrombosis in the control group,difference was statistically significant(P<0.05).Conclusion B-ultrasound guidance combined with MST for catheterization in elbow can reduce the incidence of PICC of tumor patients,prolong the use time of catheterization,reduce unplanned extubation rate,and improve the safety of patients with catheter.
