摘　　要：目的探讨健康体检人群幽门螺杆菌(HP)感染对血脂代谢的影响。方法收集2015年8月至2017年5月行HP及血脂检测的健康体检者2035例,所有体检者均进行HP和血脂指标(包括TC、TG、HDL-C、LDL-C)的检测。观察HP感染与血脂代谢的关系。结果 2035例受检者中,HP阳性率为38.0%,其中男性阳性率为42.9%,高于女性的35.4%,差异有统计学意义(P<0.001)。HP阳性者HDL-C水平为(1.37±0.03)mmol/L,低于HP阴性者的(1.41±0.02)mmol/L,LDL-C为(3.13±0.04)mmol/L,高于HP阴性者的(3.08±0.05)mmol/L,差异均有统计学意义(P<0.001)。HP阳性者HDL-C降低和LDL-C升高的比例分别为18.6%、34.1%,高于HP阴性者的15.1%、29.3%,差异有统计学意义(P<0.05)。HP阳性者与阴性者间的TC、TG水平及TC、TG升高比例差异无统计学意义(P>0.05)。结论 HP感染在男性体检者中发生率较高,且与血脂代谢异常有关,特别是与HDL-C降低及LDL-C升高关系更密切。Objective To investigate the effect of Helicobacter pylori(HP)infection on blood lipid metabolism in healthy physical examination people.Methods A total of 2035 healthy persons who underwent HP and blood lipid tests from August,2015 to May,2017 were collected.All the subjects were tested for HP and blood lipid indexes(including TC,TG,HDL-C and LDL-C).And relationship between HP infection and blood lipid metabolism was observed.Results Among 2035 people,the positive rate of HP was 38.0%,the positive rate of HP in males was 42.9%,which was higher than that(35.4%)in females,difference was statistically significant(P<0.001).In HP positive people,HDL-C level was(1.37±0.03)mmol/L,which was lower than that(1.41±0.02)mmol/L in HP negative people,and LDL-C level was(3.13±0.04)mmol/L,higher than that(3.08±0.05)mmol/L in HP negative people,difference was statistically significant(P<0.001).The proportion of decrease of HDL-C and increase of LDL-C in HP positive people was 18.6%and 34.1%,respectively,higher than that in HP negative people which was 15.1%and 29.3%,respectively,difference was statistically significant(P<0.05).There was no significant difference in the TC and TG levels,and the proportion of increase of TC and TG between the HP positive people and the HP negative people(P>0.05).Conclusion The incidence of HP infection in male with healthy physical examination is higher,and it is related to abnormal metabolism of blood lipids,especially with the decrease of HDL-C and the increase of LDL-C.
