比阿培南治疗重症监护病房细菌感染的多中心临床研究

摘　　要：目的评估比阿培南治疗重症监护病房(intensive care unit,ICU)细菌感染的有效性和安全性。方法采用多中心、回顾性研究设计,自2013年1月1日至10月1日对全国17个重症监护病房应用阿培南的805例患者进行回顾性分析。纳入标准为高度怀疑细菌感染,且连续应用比阿培南3d以上的成年ICU患者。观察指标为应用比阿培南后患者的临床疗效和细菌学疗效,同时统计不良反应情况。结果应用比阿培南治疗,患者的临床疗效总有效率为85.19%(650/763),实验室感染指标、病情严重程度、器官功能、退热时间、呼吸机使用时间、ICU住院时间均较前好转。细菌总清除率为84.17%(452/537),根据药敏结果,比阿培南的敏感性和耐药性均不差于亚胺培南和美罗培南。不良反应发生率为1.06%,其中包括皮疹/皮肤瘙痒8例,恶心7例,呕吐3例。结论应用比阿培南治疗ICU细菌性感染能得到较好的临床疗效和细菌学清除效果,且并发症低。Objective To evaluate the efficacy and the safety of biapenem in the treatment of bacterial infection in intensive care unit(ICU).Methods This study was a retrospective multicenter research.The 805 patients who were suspected bacterial infection from 17 ICUs in China were received biapenem during Janary1 to October 1 in 2013.The inclusion criterion was adults patients who were highly suspected bacterial infection and in the treatment of biapenem more than three days in ICU.The clinical efficacy,bacterial eradication rates and drug-related adverse reactions were evaluated.Results After the treatment of biapenem,the effective rate was 85.1 9%(650/763),and the experimental inflectional index,the severity of illness,the functions of organs,the fever clearance time,the length of the mechanical ventilation and ICU stays all improved.The bacterial clearance rate was 84.1 7%(452/537).According to the drug susceptibility,neither the drug sensibility nor resistance of biapenem were inferior than imipenem and meropenem.The rate of adverse drug reaction was only 1.0 6%.The adverse reactions were the rash or itchy skin,nausea and vomiting and the case numbers were respectively eight,seven and three.Conclusion Biapenem was satisfactory in the treatment of bacterial infection in ICU in view of the clinical efficacy,bacterial clearance and safety.
