摘  要:目的探讨早泄(PE)与去甲肾上腺素(NE)、5-羟色胺(5-HT)浓度等血中单胺类神经递质间的关系。方法招募我院泌尿外科门诊首诊终身性PE患者75例为试验组,同期广告招募健康男性64例为对照组。试验组患者给予盐酸舍曲林治疗8周,对照组不给予治疗,治疗期间两组均保持平常性生活规律不变。测定两组研究对象治疗前后IELT、CIPE-5评分、NE与5-HT浓度。结果试验组中8例患者因治疗过程中出现严重不良反应或失访退出本研究,共完成研究67例。试验组治疗前NE指标显著高于对照组,治疗后显著降低,与对照组比较无显著差异(P>0.05);试验组治疗前5-HT指标显著低于对照组,治疗后5-HT指标显著上升,但仍明显低于对照组(P<0.05)。治疗后试验组5-HT与IELT呈正相关关系(r=0.385,P<0.05),对照组5-HT与IELT无明显相关(r=0.0.867,P>0.05)。对于PE,血浆NE的诊断敏感度为87.5%,特异度为96.3%,5-HT的诊断敏感度为92.2%,特异度为93.0%。结论血浆高浓度NE及低浓度5-HT是PE发病的生物学指标,可作为临床诊断PE的实验室指标,指导临床工作。Objective To investigate the correlation between premature ejaculation(PE)and monoamine neurotransmitters in blood such as norepinephrine(NE)and 5-hydroxytryptamine(5-HT)concentrations.Methods 75 patients with life-long PE who were firstly diagnosed in the department of urinary surgery in our hospital were enrolled in the study group.In the same period,64 healthy males were recruited by advertisement and were selected as in the control group.The study group was given sertraline hydrochloride for 8 weeks,and the control group was not given any treatment.During treatment,both groups maintained the same regular pattern of sex life.The levels of IELT,CIPE-5,NE and 5-HT concentrations before and after treatment in both groups were measured.Results In the experiment group,8 patients were withdrawn from the study due to serious adverse reactions during treatment or lost to follow-up.A total of 67 cases were completed.The NE indexes in the study group before treatment was significantly higher than those in the control group,but decreased significantly after treatment.There was no significant difference compared with the control group(P>0.05);the 5-HT index in the experiment group before treatment was significantly lower than that in the control group.After treatment,5-HT index was increased significantly,but was still significantly lower than that in the control group(P<0.05).After treatment,there was a positive correlation between 5-HT and IELT in the experiment group(r=0.385,P<0.05).There was no significant correlation between 5-HT and IELT in the control group(r=0.0867,P>0.05).For PE,the diagnostic sensitivity of plasma NE was 87.5%and the specificity was 96.3%.The diagnostic sensitivity of 5-HT was 92.2%and the specificity was 93.0%.Conclusion The high concentration of plasma NE and low concentration of 5-HT are biological indicators of PE,which can be used as a laboratory index for clinical diagnosis of PE,so as to guide the clinical work.
