抚触结合中医小儿推拿对新生儿黄疸的效果

摘　　要：目的 探讨抚触结合中医小儿推拿治疗新生儿黄疸的临床效果。方法 将120例新生儿黄疸患儿随机分为试验组和对照组，各60例。对照组采用常规护理、间歇蓝光和茵栀黄口服液治疗；试验组在对照组的基础上加用抚触和中医小儿推拿护理，比较两组护理前后黄疸指数、黄疸持续时间及血清总胆红素，并观察比较两组护理期间胎便，是否有发热、腹泻和皮疹等。结果 两组治疗前后经皮黄疸指数及血清总胆红素差异均无统计学意义（均P＞0.05），但试验组黄疸持续时间较对照组缩短。试验组胎便排便次数较对照组明显增多（P＜0.05），而患儿发热、腹泻和皮疹等较对照组明显减少（均P＜0.05）。试验组护理后血清胆红素和黄疸指数较对照组降低，尤其黄疸消退时间缩短1.32 d，P=0.062。结论 抚触结合小儿推拿护理可以缩短新生儿黄疸治疗周期，并可促进新生儿排便以及减少发热、腹泻和皮疹等症状，值得临床推广应用。Objective To explore the clinical effect of touching combined with pediatric massage of traditional Chinese medicine for neonatal jaundice.Methods A total of 120 term infants were selected and randomly divided into an experimental group and a control group,60 cases for each group.The control group were routinely cared and treated with intermittent blue-ray and orally took Yinzhihuang liquid;in addition,the experimental group were given touching and pediatric message of traditional Chinese medicine.The jaundice index,the duration of jaundice,and serum total bilirubin were compared between the two groups before and after nursing care.The meconium and whether the infants had fever,diarrhea,skin rash,etc.were observed and compared between these two groups.Results There were no statistical differences in jaundice index and serum total bilirubin between before and after the treatment in both groups(P>0.05),but jaundice duration was shorter in the experimental group than in the control group.The frequency of defecation was higher and the incidences of fever,diarrhea,and skin rash were lower in the experimental group than in the control group(P<0.05).After the nursing care,the jaundice index and serum total bilirubin were lower and the jaundice disappeared time(P=0.062)was 1.32 d shorter in the experimental group than in the control group.Conclusions Touching combined with pediatric massage for neonatal jaundice can shorten the treatment course,promote neonatal defecation,and reduce the symptoms such as fever,diarrhea,and skin rash.Therefore,it is worth being clinically generalized.
