摘  要:探讨外周血FcγRⅡ表达水平与儿童特发性血小板减少性紫癜(ITP)的相关性。方法 采用流式细胞技术检测40例ITP患儿以及14例健康儿童外周血单个核细胞FcγRⅡ表达,病例组根据血小板水平及临床症状分为单用糖皮质激素组(GC组)、单用静脉丙种球蛋白组(IVIG组)及糖皮质激素联合静脉丙种球蛋白组(GC+IVIG组),治疗完成后第2天使用同样的方法检测外周血单个核细胞FcγRⅡ表达。结果 ITP患儿治疗前外周血单个核细胞FcγRⅡ表达(25.63±9.74)%明显高于对照组健康儿童(10.75±6.16)%,差异有统计学意义(P<0.01);GC组和GC+IVIG组免疫治疗后外周血单个核细胞FcγRⅡ表达均较治疗前升高[(27.78±9.85)%比(17.43±5.76)%、(31.84±10.09)%比(27.07±10.36)%],差异有统计学意义(P<0.05),而IVIG组免疫治疗后与治疗前比较未见明显改变[(25.50±8.92)%比(27.13±6.72)%],差异无统计学意义(P>0.05)。结论 ITP患儿外周血单个核细胞FcγRⅡ表达变化与机体免疫功能紊乱相关,GC及IVIG可能通过不同途径影响血小板的破坏从而起到治疗效果。检测外周血单个核细胞FcγRⅡ表达在儿童ITP的诊断和治疗中具有重要意义。To explore the correlation between the expression of FcγRⅡin peripheral blood and idiopathic thrombocytopenic purpura(ITP)in children.Methods 40 children with ITP and 14 healthy children were recruited into this study.The expression of FcγRⅡon mononuclear cells in peripheral blood were detected by flow cytometry.Based on platelet level and clinical symptoms,case group was divided into glucocorticoid treatment group(GC group),intravenous immunoglobulin treatment group(IVIG group),glucocorticoid and intravenous immunoglobulin treatment group(GC+IVIG group).On the second day after the end of immunotherapy,the expression of FcγRⅡwas detected by the same method.Results The expression of FcγRⅡof children with ITP before immunotherapy[(25.63±9.74)%]was significantly higher than that of healthy children[(10.75±6.16)%](P<0.01).The expressions of FcγRⅡof GC group and GC+IVIG group after immunotherapy were higher than those before immunotherapy[(27.78±9.85)%vs.(17.43±5.76)%,(31.84±10.09)%vs.(27.07±10.36)%](P<0.05);while it did not change significantly after treatment in IVIG group[(25.50±8.92)%vs.(27.13±6.72)%](P>0.05).Conclusion The change of FcγRⅡexpression of children with ITP is related to immune dysfunction,GC and IVIG may affect the destruction of platelets in different ways,thus achieving therapeutic effect.Therefore,testing of FcγRⅡon mononuclear cells in peripheral blood plays an important role in the diagnosis and treatment of ITP in children.
