伴感染的先天性中外耳畸形手术切口设计

摘　　要：目的介绍推广一种伴感染的先天性中外耳畸形手术切口设计方法。方法回顾性分析采用新的切口设计法处理11例(11耳)伴感染的先天性中外耳畸形病例,进行预制再造耳垂、病灶清理、外耳道再造或扩大成形、鼓室成形或改良乳突根治术,观察手术可操作范围、后期耳廓再造条件保存情况(包括皮肤组织完整性及血供状况、再造耳垂保留情况等)与外耳道再造效果。结果11例(11耳)均顺利完成手术,病灶清理干净,无复发,后期耳廓再造条件保存好、再造外耳道形态好,无并发症。结论新的切口设计法是伴感染的先天性中外耳畸形患者优良的手术切口设计,值得推广应用。Objective To introduce a method of surgical incision design for patients of Congenital Malformation of Outer and middle Ears(CMOME)with infection.Methods a retrospective analysis of 11 cases(11 ears)of CMOME undergone a new incision design,observation of the operative scope and preservation of skin subcutaneous tissue integrity,vascular pedicle,reconstructed ear lobe,and effect of the reconstructed external auditory meatus after operation of debridement of focus,reconstruction or enlargement of external auditory meatus,tympanoplasty or modified mastoidectomy.Results the operation was completed successfully in 11 cases(11 ears).The focus was cleaned up,no recurrence occurred,the condition of auricle reconstruction was preserved well and the shape of the external auditory meatus was good.Conclusion the new incision design is an excellent for patients of CMOME with infection,and it is worth popularizing and applying.
