摘　　要：目的分析骨桥应用效果,探索其临床适应症。方法植入骨桥的4例双侧听力障碍患者(先天性中外耳畸形2例,镫骨畸形1例,混合性聋1例),对比助听前后语频段0.5k-4k Hz平均纯音气导听阈及安静与噪声环境下65d BSPL句子识别率,评估骨桥应用效果。结果 4例患者均顺利完成手术,无并发症。助听后平均纯音气导听阈改善分别为28.8d B(前66.3d B/后37.5d B),45d B(前67.5d B/后22.5d B),97.5d B(92.5d B/后-5d B),30d B(前66.3d B/后36.3d B),助听后65d BSPL安静环境下句子识别率分别为80%(前0/后80%),100%(前6%/后100%),100%(前32%/后100%),100%(前64%/后100%),噪声环境下(10d BSNR)分别为92%,88%,86%,88%。结论骨桥植入对先天性中外耳畸形、镫骨畸形及混合性聋患者均能获得明显听力改善、达到日常交流水平,是一种良好的听力解决方案。Objective To report the efficacy of bone bridge(BB)and discuss its clinical indications.Methods Four patients with bilateral hearing impairment(2 with congenital deformation of middle and outer ears,1 with stapes malformation and 1 with mixed deafness)were included in the study.Average pure tone hearing thresholds at 0.5 k-4 kHz before and after BB were compared,and sentence recognition scores at 65 dB SPL in quiet and noisy environment as well as daily communication were assessed.Results Surgery was successful in all patients with no complications.After BB activation,average air conduction pure tone thresholds or conditioned reflex audiometry improved by 28.8 dB(from 66.3 dB HL to 37.5 dB HL),45 dB(from 67.5 dB HL to 22.5 dB HL),97.5 dB(from 92.5 dB HL to-5 dB HL)or 30 dB(from 66.3 dB HL to 36.3 dB HL)for the four patients,respectively,with significant improvement of communication.The sentence recognition score at 65 dB SPL increased from 0%to 80%,6%to 100%,32%to 100%and 64%to 100%in quiet,and to 92%,88%,86%and 88%in noise(SNR=10 dB)for the four patients,respectively.Conclusion Bone bridge implant is a good hearing solution for patients with congenital deformation of middle and outer ears,stapes malformation or mixed deafness.The patient can acquire significant improvement in hearing and daily communication.
